Face lifting without fillers means working on the tissue you already have instead of adding product to it. Dr Sin Yong leads with his own protocols, the VF Lift – Vertical Facelift and Time Freeze Laser LCLR®. Nothing is injected, so there is no filler to overfill, migrate or form lumps. This guide explains why lifting comes first, which protocol suits which concern, and where injectables still have a place.
Part of Lifting and sagging →
Face lifting without fillers starts from a simple observation: most faces that look tired or sunken have not lost as much volume as they appear to. The tissue has moved. Facial fat sits in separate compartments, and with age these deflate and slide downward and inward (Rohrich 2007). Tissue that once sat over the cheekbone gathers lower down. It shows as a deeper smile line, a jowl along the jaw, or a shadow beside the mouth.
A face ages in several layers at once:
Filler answers one of these layers: it adds volume. It does not return tissue that has descended to where it was. Adding volume to tissue that is already falling adds weight to the fall, which is why a face filled to correct sagging can end up heavier rather than lifted.
So the first question at assessment is not which product, but what has happened. Has the tissue moved, emptied, or both? Movement and laxity are answered by lifting. Hollowing is answered first by the Restore mode of the VF Lift, which works on your own fat, and filler is considered only where that is not enough. The wider options for sagging are compared on face lifting treatment and in the non-surgical facelift guide.
Overfilling is rarely one treatment. It builds when new filler is placed on top of filler that has not gone. An MRI review of 33 patients found hyaluronic acid filler still present in the mid-face of every one, including people treated many years earlier (Master 2024). Each top-up adds to what is already there, and hyaluronic acid also draws in water.
The usual pattern, described in the medical literature as facial overfilled syndrome, has a few common causes: product placed to chase a line or shadow rather than to restore structure, product in the wrong plane, and a request for more volume each time the face looks tired (Lim 2024). Sagging is often the reason the face looked tired in the first place, so more filler does not solve it.
Lifting your own tissue avoids this by design: nothing is injected, so there is nothing to accumulate, move or form a lump. If it has already happened, the overfilled face is assessed with ultrasound first, as set out on pillow face treatment. Migrated product is covered area by area on migrated filler by area, and the early signs on filler migration signs.
“Most sagging is tissue that has moved, not volume that has gone. I lift your own tissue first, and keep filler for the few things lifting cannot do.”
Dr Sin YongOn lifting before adding volume
The VF Lift – Vertical Facelift is Dr Sin Yong's protocol on the CLASSYS Volnewmer, a 6.78 MHz monopolar radiofrequency platform with water cooling. It heats the dermis and the fibrous layer beneath it as a volume, planned zone by zone along vertical vectors. It uses no focused ultrasound, no threads and no injected product. He has presented the protocol to physicians at the CLASSYS APAC Symposium. It has three modes, set for each zone after assessment.

Tightens and lifts loose skin along vertical vectors. It is directed at jawline definition, the nasolabial folds, a double chin and the overall shape of the face.
Flattens areas that pouch or bulge, such as a puffy lower face or the fullness beside the nasolabial fold. It does not affect facial hair, so a beard is not a reason to avoid it.
The mode Dr Sin Yong values most. It is customised to hollow areas and used to stimulate the patient's own fat to regenerate, so any change builds gradually rather than appearing on the day. He uses it over:
Because the volume is your own tissue, there is no product to overfill or migrate. How much change is seen varies between people and with age. The VF Lift is not used with a pacemaker or other implanted electronic device, metal implants near the field change the plan, and a very thin face is treated with restraint.
Time Freeze Laser LCLR® is Dr Sin Yong's laser lifting protocol on a long-pulsed 755 nm and 1064 nm platform. The two wavelengths have different absorption profiles and are used together, delivering heat at several depths of the skin. Collagen contracts during the session and remodels afterwards. No redness, no downtime.
LCLR® stands for Laser Collagen Lipo Remodelling, and the name describes its two actions:
The Sculpting mode has a second use that matters on a page about fillers. There is no dissolving enzyme for collagen stimulators such as PCL, PLLA, PDLLA or CaHA. When they have been overdone, the excess and the collagen they produced have to be remodelled rather than dissolved. Dr Sin Yong maps each deposit with ultrasound first, then uses the Sculpting mode to take away the excess. Not every laser can do this.
How he chooses between the two protocols is easy to state. A lean or hollow face is planned with the VF Lift, because focused energy that reduces fat would deepen the hollows. A puffy or heavier face, with sagged fat or settled product, is planned with Time Freeze Laser LCLR®. Many faces have both, and the two are then sequenced. Time Freeze suits mild to moderate laxity; freshly tanned skin is not treated, and marked skin excess is referred.
Lifting without fillers is planned concern by concern, because each area sags for its own reasons. The table sets out which protocol usually leads and why. Most faces have more than one finding, and the examination decides the mix.
| Concern | What usually drives it | Usually led by | Why |
|---|---|---|---|
| Jowls | Lax tissue at the jaw edge, a compact jowl fat pad, or both | VF Lift Lift mode; Time Freeze Sculpting mode for fat | Lift answers laxity; the Sculpting mode answers a fat pad that lifting would only move |
| Nasolabial folds | The cheek above descending onto the fold | VF Lift Lift mode, Sculpt for fullness beside the fold; Time Freeze | Works on the mid-face that forms the fold, from above, rather than filling it from below |
| Marionette lines | A descended jowl folding against the chin, often with muscle pull | Lifting first; botulinum toxin for the mouth-corner muscle | Filling the sides of the line adds weight where the jowl gathers |
| Sagging cheeks | Mid-face descent, often with lateral hollowing | VF Lift: Lift for descent, Restore for the lateral cheek | One protocol can lift one zone and restore the next |
| Hollows: temples, forehead, under-eyes, lateral cheeks, prejowl sulcus | Deflated fat compartments | VF Lift Restore mode | Used to stimulate your own fat to regenerate; filler is considered only if needed |
| Double chin | Submental fat, skin laxity, or a short chin | Time Freeze Sculpting mode or VF Lift | Fat and laxity are treated from the surface; a short chin is a structural question |
| Neck | Loose, crepey skin, fat under the jaw, or platysma bands | VF Lift and Time Freeze, which Dr Sin Yong prefers to Thermage here | Conservative heating over thin neck skin; bands are a muscle question |
| Brow and upper eyelids | Lax lid skin, a descended brow, or a weak lid muscle | Time Freeze periorbital zone; VF Lift for droopy upper eyes | Tightens lax skin; true ptosis of the lid muscle is referred |
Three patterns recur. Where the problem is descent, lifting comes first. Where fat or settled product makes an area heavy, the Sculpting mode of Time Freeze leads. Where an area has genuinely emptied, the Restore mode of the VF Lift is used before any filler is considered. Hooded or heavy upper lids are covered in more detail on non-surgical eyelid lift.
Dr Sin Yong prefers energy-based treatment for almost every concern that injectables are used for, because it adds no product that can cause pillow face, migration or lumps. That is a preference, not a rule against injectables. Botulinum toxin and skin boosters remain part of his practice, and he still uses filler as needed, but sparingly.
Lifting does not relax a muscle. Where a line is made by movement, such as frown lines, forehead lines or crow's feet, or where a muscle pulls the mouth corners down or makes the jaw wide, botulinum toxin is the tool for that part of the problem. It is a prescription medicine, given after a doctor's assessment.
Skin boosters are aimed at skin quality: hydration, fine texture and thin, crepey skin. They do not lift and are not a volume treatment, so they sit alongside lifting rather than competing with it.
Filler is still used where it is needed, mostly where the missing structure is bone rather than soft tissue and energy cannot build it, for example:
When filler is used, it is used sparingly, placed for structure, and recorded. Where filler is already present, ultrasound shows what is there before anything new is planned. Hyaluronic acid can be dissolved with hyaluronidase; collagen stimulators are remodelled with the Sculpting mode of Time Freeze Laser LCLR®. The types of filler and where each is used are compared on dermal fillers.
Lifting your own tissue has limits, and it is better to say so before treatment than after. Neither protocol removes skin or repositions tissue the way a surgical facelift does. Dr Sin Yong does not perform surgery; these findings are referred to a plastic surgery or oculoplastic specialist:
Afterwards, the VF Lift usually leaves redness, warmth and mild swelling that settle on their own. Time Freeze Laser LCLR® leaves no redness and no downtime, and sun protection matters after both. Fees are not published; a written quote follows the examination, as how fees are quoted explains. The visit itself is described on the consultation process.

The VF Lift – Vertical Facelift uses monopolar radiofrequency to lift lax tissue, flatten puffy areas and, in its Restore mode, stimulate your own fat in hollow areas. Time Freeze Laser LCLR® works on dermal laxity and compact fat. Nothing is injected. Marked skin excess is beyond any non-surgical treatment and is referred to a plastic surgery specialist.
They do different jobs, so it is not a ranking. Lifting works on tissue that has loosened or moved down. Filler adds volume. Because most sagging is descent rather than loss, Dr Sin Yong starts with lifting and uses filler as needed, but sparingly, mainly for structure that energy cannot build, such as a short chin or the nose profile. An examination shows which applies to your face.
Here the first option is the Restore mode of the VF Lift – Vertical Facelift, used to stimulate your own fat to regenerate over the temples, forehead, under the eyes, lateral cheeks and prejowl sulcus. Any change builds gradually and varies between people and with age. A hollow that is mainly bony is not reached by energy, and filler is then discussed.
Not because of the switch itself. Filler is not removed by lifting, and stopping top-ups does not suddenly empty the face. If existing filler has made areas heavy, ultrasound shows what is there; hyaluronic acid can be dissolved selectively and in stages, and support is then planned with the VF Lift Restore mode rather than more product.
Not always. Recent filler or threads in the treatment plane are timed around rather than heated over. Where hyaluronic acid filler is causing puffiness, it is usually dissolved first. Where a collagen stimulator has been overdone, the Sculpting mode of Time Freeze Laser LCLR® is used on the excess before heat is added. Ultrasound decides which applies.
It depends on the protocol. After the VF Lift – Vertical Facelift, redness, warmth and mild swelling are expected and settle on their own; rarely a blister or a patch of altered sensation needs review. Time Freeze Laser LCLR® has no redness and no downtime. With either, sun protection matters, and any change builds gradually rather than on the day.
