Liposuction removes fat but cannot make skin shrink to fit. When the skin over a treated area stays loose, or the contour is uneven, Dr Sin Yong assesses what is laxity, what is residual fat and what is a dent, then treats laxity without further surgery with VF Lift – Vertical Figure Lift and Time Freeze Laser LCLR®.

Short answer
Loose skin after liposuction happens when fat has been removed but the skin over it has not retracted enough to fit the new contour, which is more likely with age, larger volumes, thinner skin, previous weight loss or pregnancy. Dr Sin Yong first separates laxity from residual fat, unevenness and dents, then treats laxity without further surgery: VF Lift – Vertical Figure Lift for skin that has lost its recoil, Time Freeze Laser LCLR® where laxity and a thin residual fat layer coexist, and the 4K Body Lift for thicker residual pockets. Marked skin excess and true dents are referred to a plastic surgeon. He assesses and treats this personally at his clinic on Orchard Road, Singapore.
Key takeaways
Liposuction removes fat; how well the skin retracts depends on age, skin quality, the volume removed and history of weight change.
Early swelling and firmness are part of recovery, so the final contour is judged after several months, not weeks.
Loose skin, residual fat, uneven contour and dents are four different problems with four different answers.
VF Lift – Vertical Figure Lift is Dr Sin Yong’s body protocol for skin that has lost its recoil; on the face and under the chin, VF Lift – Vertical Facelift.
Time Freeze Laser LCLR® tightens and refines a thin residual fat layer in the same session.
Microliposuction and revision liposuction are surgery, regulated in Singapore, and referred to an accredited surgeon when needed.
“Liposuction takes the fat out. Whether the skin follows is a separate question, and it is the one most people are left asking.”
— Dr Sin Yong
In brief
Saggy skin after liposuction is skin that has not shrunk to fit the smaller contour once the fat has gone. Skin tightening after liposuction starts by separating laxity from residual fat and dents. Dr Sin Yong treats mild to moderate laxity without surgery, with VF Lift – Vertical Figure Lift and Time Freeze Laser LCLR®, and refers marked excess skin.
Why skin can stay loose after liposuction
Liposuction removes fat through a cannula. It does not remove skin, so the skin over the treated area has to retract to fit the new, smaller contour. How well it does that depends on its elastic recoil, which falls with age and sun damage and is reduced by stretch marks, previous weight loss and pregnancy. The larger the volume removed, the more the skin is asked to retract.
When the skin cannot follow, the area can look loose, crepey or folded, most often on the lower abdomen, inner thighs, upper arms and, after facial liposuction, under the chin and along the neck.
What it is: Skin that has not retracted to fit the new contour after liposuction or microliposuction, sometimes with residual fat, unevenness or a dent alongside it.
Layer: The dermis and its elastic recoil, which fall with age, sun damage, stretch marks, previous weight loss and pregnancy; the fat layer beneath is assessed separately.
Who suits: Laxity that is confirmed as laxity on examination: VF Lift – Vertical Figure Lift on the body, VF Lift – Vertical Facelift on the face and under the chin, and Time Freeze Laser LCLR® where a thin residual fat layer coexists.
What it is not: Not a treatment for marked skin excess or a true dent, which are referred to a plastic surgery specialist; not revision liposuction, which is surgery and regulated in Singapore.
What is normal in recovery, and when to look again
Swelling, bruising, numbness and firmness are expected after liposuction, and the tissue continues to settle for several months. Some unevenness in the first weeks reflects swelling rather than the final contour. For that reason, laxity and contour are judged once recovery has settled, and your surgeon’s own follow-up comes first.
Some signs are not part of normal recovery and should be seen by your surgeon promptly: a growing fluid swelling, increasing pain, redness or warmth, fever, or skin that darkens or breaks down.
Saggy skin after liposuction: who is more likely to get it, and can it firm up again?
Saggy skin after liposuction is more likely when the skin had little recoil to begin with. The main predictors are age, sun damage, stretch marks, a history of large weight change or pregnancy, and the amount of fat removed relative to the quality of the skin over it. Younger skin with good elasticity usually retracts on its own; skin that was already loose before the procedure often does not.
What lowers the risk
An assessment of skin quality before the procedure, so the plan matches what the skin can do. Where skin support is weak, removing more fat is not always better, because it can leave rippling or sagging.
A stable weight before and after liposuction.
Following the aftercare given by the doctor who performed the liposuction, including compression garments where advised.
Sun protection and not smoking, both of which affect skin quality and healing.
Can it firm up again?
Partly, and within limits. Some skin retraction continues while swelling settles, which is why the contour is judged once recovery is complete. After that, skin that has lost its recoil can be firmed with energy-based tightening, which heats the dermis so that collagen contracts and remodels. It suits mild to moderate laxity. Skin that hangs as an apron is beyond what any energy device reaches and is a question for a plastic surgery specialist.
In the long run
Liposuction removes fat cells from the treated area, but it does not stop the remaining fat from enlarging with weight gain. A randomised trial in non-obese women found that body fat returned after small-volume liposuction and was redistributed from the thighs to the abdomen. Skin also keeps ageing, so laxity can appear later even where it was not there at first. Weight stability and sun protection matter after liposuction as much as before it. How weight change affects skin is covered on loose skin after weight loss.
Four problems, four different answers

Problem
What it looks like
What tends to help
Loose skin (laxity)
Skin that folds, crepes or hangs once the fat beneath has gone
VF Lift – Vertical Figure Lift; Time Freeze Laser LCLR®; surgery for marked excess
Residual fat
A pocket or bulge that remains in or next to the treated zone
Time Freeze Laser LCLR® for superficial fat; 4K Body Lift for thicker pockets; revision liposuction
Uneven contour
Ripples, ridges or a step between treated and untreated areas
Assessed individually; energy-based refinement where fat is the cause
Dents and over-resection
A hollow where too much fat was removed
Not more fat removal. Support and restoration; fat grafting by a surgeon where needed
A systematic review of isolated aesthetic liposuction reported skin irregularities in between 0.12% and 3.36% of cases depending on technique.1 They are uncommon, but they matter to the person who has them, and they are corrected differently from laxity.
Loose skin, residual fat, an uneven contour and a dent are four different problems with four different answers. Schematic, not to scale.
Illustrative image, not a patient. Laxity, residual fat, uneven contour and dents are separated before anything is planned.
VF Lift for skin that has lost its recoil
VF Lift – Vertical Figure Lift is the body version of Dr Sin Yong’s VF Lift protocol. It runs on Volnewmer 6.78 MHz monopolar radiofrequency, heating the dermis and the fibrous septa beneath it as a volume, and is planned zone by zone along vertical vectors. It is the protocol he uses where the skin has lost its recoil: the lower abdomen, flanks, arms, thighs and buttocks after liposuction or weight loss.
Response builds gradually as collagen remodels, and it varies. It suits mild to moderate laxity; a large apron of excess skin is beyond what any energy device reaches and is referred for surgical opinion.
Time Freeze Laser LCLR® where laxity and residual fat coexist
Time Freeze Laser LCLR® is Dr Sin Yong’s dual-wavelength laser protocol. The collagen arm tightens the skin at two depths, and the lipo-remodelling arm refines superficial fat in selected zones through a Sculpting mode. After liposuction it is used where loose skin sits over a thin layer of residual fat, so both are addressed in the same session, with no downtime.
For thicker residual pockets, the 4K Body Lift (radiofrequency microneedling) is used, alone or with VF Lift, before any discussion of further surgery.
Skin tightening after liposuction: during the procedure, afterwards, or surgical
Skin tightening after liposuction can happen at three points, and each belongs to a different setting. Some liposuction teams add energy beneath the skin during the procedure itself. Surface treatments come later, once recovery has settled. Where the excess is marked, skin is removed surgically.
When
How it works
Who provides it
What decides it
During liposuction
Radiofrequency or plasma energy applied beneath the skin through the liposuction access, to encourage contraction as fat is removed
The liposuction team, in a hospital or approved surgical centre
Skin quality judged before the procedure; part of the surgical plan
Laxity confirmed on examination, once healing is complete
Surgical skin removal
Abdominoplasty, or an arm or thigh lift, removes the excess skin
A plastic surgery specialist
A large apron or fold of excess skin, or slack abdominal muscles
A 2013 study that added radiofrequency heating during abdominal liposuction measured a reduction in skin surface area, which is why some teams combine the two. That is a surgical decision made before the operation. At the other end, a review of more than 300 consecutive abdominal contouring patients found that liposuction alone suited most, while those with skin laxity or slack abdominal muscles needed open surgery to reduce skin. Dr Sin Yong works in the space between: he treats laxity from the surface and does not perform liposuction.
How the timing is decided
There is no fixed waiting period. Surface tightening starts once swelling and firmness have settled and the doctor who performed your liposuction is satisfied with healing. Progress is reassessed at each review, and the plan is adjusted, or stopped if the honest answer is surgery. The skin tightening treatment Singapore guide compares radiofrequency, ultrasound and laser by depth.
After face, chin or jowl liposuction
Facial and submental liposuction are popular because the areas are small. The same rule applies: if the skin under the chin or along the jaw does not retract, the neck can look loose or banded, and over-thinned fat can leave the jawline looking hollow or uneven.
On the face Dr Sin Yong uses VF Lift – Vertical Facelift, which lifts, sculpts residual puffiness and restores lean areas in one plan, and Time Freeze Laser LCLR® for tightening with point-zone refinement under the chin. Where fat has been over-removed and the area is hollow, he does not remove more; the plan turns to support instead.
Microliposuction, revision liposuction and surgery
Microliposuction is liposuction on a small scale, using fine cannulas for small areas such as the chin, jowls or a residual pocket. In Singapore it is regulated in the same way as any liposuction: only doctors accredited under the Ministry of Health’s framework may perform it, in a hospital or an approved Ambulatory Surgical Centre, with a seven-day cooling-off period, and certain cases, including repeat procedures over the same region, must be done as inpatients.3,4
Patients sometimes ask about “micro laser liposuction”. That is also an invasive procedure: a laser fibre is passed under the skin through a cannula. Time Freeze Laser LCLR® is different. It is applied from the surface, with no cannula, no anaesthesia and no incisions, and acts on superficial fat and skin rather than extracting fat.
Where residual fat is large and well defined, where a dent needs fat to be moved into it, or where there is a large excess of skin, revision liposuction, fat grafting or a tummy tuck are surgical decisions. Dr Sin Yong does not perform liposuction; when surgery is the right answer he says so and refers you to an accredited surgeon.
When fat was moved to the face as well
Liposuction is often combined with fat transfer: fat taken from the abdomen or thighs is injected into the cheeks, temples or under the eyes. If the face now looks puffy, heavy or lumpy, that is a different problem from the donor site, and it is covered on fat grafting correction.

Myths we hear in clinic
“Liposuction tightens the skin.” It removes fat. Skin retraction afterwards depends on the skin itself.
“More liposuction will fix loose skin.” Removing more fat under loose skin can make laxity look worse.
“Micro laser liposuction is non-invasive.” It uses a laser fibre under the skin through a cannula. Surface laser treatments such as Time Freeze Laser LCLR® are a different thing.
What determines the fee
The fee depends on the areas treated, the number of zones, which protocols are combined and how many sessions are planned. Fees are not published, because a figure given before an examination cannot reflect your situation. A written quote is given at consultation, and the consultation also decides whether treatment is advised at all. How fees are quoted.
Three ways Dr Sin Yong uses the Time Freeze Laser LCLR®
Lifting across every zone — the face, jowls, nasolabial folds, neck and double chin are lifted with the same laser, planned zone by zone.
Same-day reduction of excess filler — where an area has been overfilled, the LCLR takes away the excess in the same session, without waiting for a dissolving injection to work.
Treating the excess in sagging areas — where sagging comes with loose fat, the laser reduces that loose fat while it lifts, which is why the jowl, the double chin and the neck respond as they do.
The loose-fat reduction is comparable in aim to micro-liposuction, but without surgery and its risks or recovery, and it comes with skin improvement and tightening that liposuction does not give. Suitability is decided at an in-person assessment. Read more on the Time Freeze Laser LCLR® page, the Sculpting mode for excess filler, and micro-liposuction vs Time Freeze Laser LCLR®.
A device is only a device: technique and experience make the difference
Dr Sin Yong explains it to patients this way. Ultherapy Prime, Thermage or any lifting machine is a tool, in the way a suture is a tool: in experienced hands a suture gives a beautiful facelift, and in inexperienced hands the same suture can leave scarring and a poor result. The device is identical; the technique and the experience are not.
The same is true of fillers. One patient looks natural and another has bulges and folds that were never there before, with the same product. Where it is placed, how deep, how much and in what sequence decide the outcome. It is never the product alone.
Dr Sin Yong sees many patients who have had Ultherapy Prime or Thermage elsewhere with little change. The device did not fail; the treatment was not customised to that face and the technique was not optimal for it. The Time Freeze Laser LCLR® is his optimised protocol, customised to each patient, and the reason people travel from Dubai, South Africa, Hong Kong and elsewhere to have their filler complications corrected and their face lifted by him. Suitability is still decided at an in-person assessment.
Frequently Asked Questions — Loose Skin After Liposuction
First find out what the sagging is. Skin that has lost its recoil is treated with energy-based skin tightening after liposuction: here, VF Lift – Vertical Figure Lift on the body or VF Lift – Vertical Facelift under the chin, and Time Freeze Laser LCLR® where a thin layer of fat remains. Residual fat, dents and a large apron of skin need different answers, and the last two are referred for a surgical opinion.
Not always. Liposuction removes fat but not skin, so the skin has to retract to the new contour. Younger skin with good elasticity usually does. Sagging is more likely with age, sun damage, stretch marks, previous weight loss or pregnancy, and when a large volume is removed beneath thin skin. A skin assessment before the procedure helps match the plan to what the skin can do, and laxity that remains after recovery can be assessed then.
Within limits, yes. Mild to moderate laxity can firm with energy-based tightening, which heats the dermis so that collagen contracts and remodels. Dr Sin Yong uses VF Lift – Vertical Figure Lift and Time Freeze Laser LCLR® for this. How much changes varies, and it is reviewed rather than promised. Skin that hangs in an apron or a fold does not tighten enough with any device and is a surgical question.
Over the long term, the treated area keeps fewer fat cells, but the cells that remain can still enlarge with weight gain, and fat can be laid down in untreated areas instead. A randomised trial found body fat redistributed from the thighs to the abdomen after small-volume liposuction. Skin also keeps ageing, so laxity can appear later even if it was not there at first. A stable weight and sun protection help, and later laxity is assessed like any other.
It varies, and no single timetable applies. Skin keeps retracting while swelling and firmness settle, so the contour is judged only once recovery is complete. What decides it is the skin’s own recoil, your age, the volume removed and your weight history. If skin tightening after liposuction is needed, the response builds gradually as collagen remodels, and progress is reassessed at each review rather than measured against a fixed date.
Because liposuction removes fat but cannot make skin shrink. If the skin’s elastic recoil is reduced by age, sun damage, stretch marks, earlier weight loss or pregnancy, it cannot follow the smaller contour and folds or hangs instead. Removing a large volume under thin skin makes this more likely. Sometimes what looks like sagging is residual fat, swelling or a dent, which is why each is assessed separately before any plan.
Liposuction on its own removes fat; it is not a skin-tightening procedure. Some liposuction teams add radiofrequency or plasma energy beneath the skin during the procedure to encourage contraction, and that is part of the surgical plan. After recovery, skin tightening after liposuction is done from the surface. Dr Sin Yong does not perform liposuction; he treats remaining laxity with VF Lift – Vertical Figure Lift and Time Freeze Laser LCLR®, and refers when surgery is the better answer.
For mild to moderate laxity, energy-based tightening is the usual route. Dr Sin Yong uses VF Lift – Vertical Figure Lift (monopolar radiofrequency) where the skin has lost its recoil, and Time Freeze Laser LCLR® where loose skin sits over a thin layer of residual fat. A large apron of excess skin is a surgical question.
Some retraction continues for several months as swelling settles, so the final contour is judged after recovery. Skin that is still loose once things have settled is unlikely to change much without treatment.
Once your surgical recovery has settled and your surgeon is happy with healing. Timing is set at assessment rather than to a fixed schedule.
Liposuction on a small scale, using fine cannulas for small areas such as the chin, jowls or a residual pocket. In Singapore it is regulated as liposuction: accredited doctors only, in a hospital or approved surgical centre, with a seven-day cooling-off period.
No. Laser liposuction, including micro laser liposuction, passes a laser fibre under the skin through a cannula. Time Freeze Laser LCLR® is applied from the surface with no cannula, anaesthesia or incisions, and acts on superficial fat and skin.
Early lumps are usually swelling and firmness that settle over months. Unevenness that remains can be residual fat, ripples from the skin not retracting evenly, or dents where too much fat was removed. Each is assessed separately.
A true dent from over-removal is not helped by removing more fat. It usually needs support or volume, and moving fat into it is a surgical decision. Dr Sin Yong will say if that is the case and refer you.
Yes, where it is mild to moderate. VF Lift – Vertical Facelift and Time Freeze Laser LCLR® are both used on the lower face and neck, chosen by whether laxity, residual fat or hollowing dominates.
No. Dr Sin Yong treats laxity and superficial fat without surgery. Liposuction, including microliposuction, is performed only by MOH-accredited doctors in approved settings, and he refers to them when surgery is the right answer.
The areas treated, the number of zones, which protocols are combined and how many sessions are planned. Fees are not published; a written quote is given after the assessment.
Face liposuction in Singapore, usually of the chin, jowls or cheeks, removes fat but does not tighten skin, so where skin elasticity is limited the area can look looser afterwards. Skin quality is assessed first, and when looseness follows liposuction, skin-tightening treatment is planned around the treated area.
Skin tightening after liposuction works best once swelling has settled, using the 4K Body Lift or the Time Freeze Laser LCLR® depending on how loose the skin is and where. Dr Sin Yong examines the skin's recoil before suggesting either, because very loose skin may still need surgery.
