Chin and jowl liposuction are popular because the areas are small and the change can be clear. When the skin under the chin does not retract to fit the slimmer contour, though, the neck can look loose, crepey or banded, and if too much fat was removed the jawline can look hollow. These are two different problems, and they are treated differently.

Short answerAfter chin or face liposuction, loose neck skin means the skin has not retracted to fit the new contour, which is more likely with age, thinner skin or a larger amount removed. Once recovery has settled, mild to moderate laxity can be treated without surgery: Dr Sin Yong uses VF Lift – Vertical Facelift to lift and tighten and Time Freeze Laser LCLR® for tightening with point-zone refinement. Where fat has been over-removed, he does not remove more; the plan turns to support.

Why the neck skin may not follow

Under the chin the skin is thin and moves constantly. After fat is removed it has to retract over a smaller volume, and how well it does depends on its elastic recoil. Age, sun damage, previous weight loss and the amount removed all reduce that recoil. Platysmal bands, the vertical cords of the neck muscle, can also become more visible once the fat that softened them has gone.

What is normal in the first months

Swelling, firmness and numbness under the chin are expected, and the skin continues to settle for several months. Small irregularities early on usually reflect swelling. Laxity and contour are judged once that has settled, and your surgeon’s own follow-up comes first.

How laxity is treated without surgery

VF Lift – Vertical Facelift runs on Volnewmer 6.78 MHz monopolar radiofrequency, planned zone by zone along vertical vectors. Under the chin and along the jaw it is used to lift and tighten, and where the face has become lean it also restores lean areas. Time Freeze Laser LCLR® tightens the skin at two depths and, through its Sculpting mode, refines any small residual fat pocket in the same session, with no downtime.

Both are gradual, and response varies. A large amount of excess skin, or prominent platysmal bands, may be better served by a surgical neck lift, and Dr Sin Yong refers when that is the honest answer. More detail is on loose skin after liposuction.

“Under the chin, the question after liposuction is rarely about fat any more. It is about the skin, and the skin needs its own plan.”

Frequently Asked Questions

Some retraction continues for several months. Skin that is still loose once things have settled is unlikely to change much on its own, but mild to moderate laxity can be treated without surgery.

Once your surgical recovery has settled and your surgeon is happy with healing; timing is set at assessment.

No. Removing more fat under loose skin usually makes laxity look worse.

That is over-removal rather than laxity. More fat removal will not help; the plan turns to support and restoring lean areas, which VF Lift is used for.

No. He treats the after-effects without surgery and refers to MOH-accredited surgeons when liposuction or a neck lift is the right answer.
