A jowl can be lax tissue, a compact fat pad, settled filler, or a mix, and Time Freeze Laser LCLR® is used for the heavy part. The laser heats the dermis along the jaw, and its Sculpting mode is directed at compact fat or product that weighs the lower face down. Dr Sin Yong tells these apart before planning. No redness, no downtime.

Laser for jowls makes sense once you know what the jowl is made of. A true jowl is descended tissue: skin, fat and the SMAS sliding below the jawline as the retaining ligaments weaken, gathering against the mandibular ligament. A jowl can also be a compact fat pad, or product from earlier filler or a collagen stimulator that has settled along the lower face. Many jowls are a mix.

A simple self-check helps. Tilt your chin down and clench your teeth: tissue that folds over the jawline is descent. Softness that stays with your head held high, and feels thick when pinched, is likely fat. Vertical bands that tighten when you grimace are platysma, a neck question rather than a jowl. The full self-sort is on jowl fat vs jowl sagging.
Dr Sin Yong examines the jawline at rest, with the teeth clenched and with the chin tilted down, and uses ultrasound where earlier filler might be adding weight. The answer decides whether Time Freeze leads, the VF Lift – Vertical Facelift leads, or the two are used together.
Time Freeze Laser LCLR® works on two parts of a heavy jowl in the same session. The long-pulsed 755 nm and 1064 nm wavelengths heat the dermis along the jaw at several depths, so collagen contracts and then remodels. The Sculpting mode is directed at compact fat that has sagged into the jowl, through controlled bulk heating, in zones Dr Sin Yong selects at assessment.
Settings are calibrated zone by zone: lifting parameters where tissue needs tightening, Sculpting-mode parameters over the fat pad, and skin-quality parameters for texture. The lean parts of the lower face are left alone, because taking fat from an area that has none to spare deepens hollows. Order matters in a mixed jowl: reducing a fat pad before tightening changes how the tissue redrapes, which is why the plan is sequenced rather than applied all at once.
Nothing is injected, so there is no filler to overfill, migrate or form lumps along the jawline. Filler placed into a jowl adds weight to tissue that is already descending. There is no redness and no downtime; a Sculpting zone can feel tender for a while. The neck and under-chin are often treated in the same session where indicated, as set out on Time Freeze Laser for the neck and double chin.
“A heavy jowl and a loose jowl can sit on the same jawline. One needs something taken away, the other needs lifting, and treating the wrong one changes very little.”

None of these is a ranking. Each answers a different kind of jowl, and many lower faces need two.
| What the jowl is made of | Usually led by | Why |
|---|---|---|
| Compact fat in a fuller lower face, with mild to moderate laxity | Time Freeze Laser LCLR® and its Sculpting mode | Tightens the dermis and targets the fat that lifting alone would move rather than reduce |
| Laxity at the jaw edge in a lean face | VF Lift – Vertical Facelift | Lifts along vertical vectors without taking fat from a face that has little |
| A mix of fat and laxity | Time Freeze and the VF Lift together | The two can be used on the same face, each on its own zone |
| SMAS laxity in a face with normal fullness | HIFU for jowls | Focused ultrasound reaches the SMAS where there is fat to spare |
| Moderate descent in skin that holds a thread | Jawline threads | Repositions tissue mechanically along a vector |
| A heavy, mobile jowl or folds of skin | Referral | A surgical opinion from a plastic surgery specialist |
A dip in front of the jowl, the prejowl sulcus, is a hollow rather than excess, and the Restore mode of the VF Lift is used there. Mouth corners pulled down by muscle answer to botulinum toxin, and a short chin may need a small amount of filler on bone. How the laser and focused ultrasound divide the lower face is set out on Time Freeze Laser LCLR® vs HIFU.
Some heavy jowls are partly product. Filler or collagen stimulator placed along the jawline or lower cheek of a face that was already full can make the lower face look heavier and the jowl more obvious. Hyaluronidase dissolves hyaluronic acid, but there is no dissolving enzyme for collagen stimulators such as PCL, PLLA, PDLLA or CaHA, or for grafted fat.
For material that cannot be dissolved, Dr Sin Yong uses the Sculpting mode of Time Freeze Laser LCLR®, without an incision. Each deposit is mapped with ultrasound first, locating it and measuring its size and depth, so that energy is directed at the excess rather than the surrounding tissue. Hyaluronic acid on the map is usually dissolved instead, and many faces need both in a planned order. He does not add more product to hide heaviness; where the lower face has lost shape, that is considered after the excess has been addressed.
A lump that is red, hot, tender or growing quickly is treated medically first. How the mode is used on nodules is set out on the Sculpting mode of Time Freeze Laser LCLR®, and the overfilled face on pillow face.
Time Freeze Laser LCLR® suits mild to moderate laxity. It does not remove folds of loose skin, and it does not reposition a heavy, mobile jowl on the day; those are assessed for a thread lift or referred to a plastic surgery specialist for a facelift opinion. It does not replace lost volume, so a jowl made more visible by a deflated mid-face may need support above it instead.
Heat carries its own effects. Tenderness where fat was targeted is expected and settles. Less often there may be small areas of crusting or temporary darkening of the skin, more likely in darker skin and after sun exposure. Rare risks include a burn or blister, lightening of the skin, scarring and more loss of fat in a zone than intended. They are reduced by assessment, settings adjusted for skin type and each zone, ultrasound mapping before filler is treated, and sun protection. WhatsApp Dr Sin Yong →or use the enquiry formIt depends on what the jowl is made of. Time Freeze Laser LCLR® tightens the dermis along the jaw and its Sculpting mode is directed at compact jowl fat, so it suits mild to moderate laxity with a heavy component. It does not remove folds of skin or reposition a heavy, mobile jowl, which are assessed for threads or a surgical opinion. How much changes varies between people.
It is not a ranking. A jowl that is heavy with compact fat suits Time Freeze Laser LCLR® and its Sculpting mode. A jowl from laxity in a lean face suits the VF Lift – Vertical Facelift. Many lower faces have both, and the two protocols can be used together on the same face, each on the zone it suits.
The Sculpting mode is reserved for zones that are too full, and lean areas are left alone, which is why zones are selected at assessment. More loss of fat in a zone than intended is a rare risk, and it should be reported if a zone looks hollower than the rest once tenderness has settled. A lean face is planned with the VF Lift instead.
Filler placed into a jowl adds weight to tissue that is already descending, and filler along the jawline of a full face can make the jowl more obvious. Where support has been lost above or in front of the jowl, that is assessed separately; the prejowl sulcus is usually planned with the VF Lift Restore mode, and filler is used as needed, but sparingly.
Yes, where indicated. The jawline, the area under the chin and the neck are often planned together so the lower face and neck stay balanced. Neck skin is thinner, so settings there are kept conservative and the thyroid is kept out of the field. Which zones are included is decided at assessment.
