Time Freeze Laser LCLR® vs Ultherapy compares two lifting treatments Dr Sin Yong performs. Ultherapy Prime images the tissue, then places micro-focused ultrasound points at set depths down to the SMAS. Time Freeze Laser LCLR® is his dual-wavelength laser protocol, heating the dermis at several depths with a Sculpting mode for excess. How much fat each zone has usually decides.

Short answerUltherapy suits structural descent at the SMAS in a face with normal fullness. Time Freeze Laser LCLR® suits laxity with a puffy or heavy lower face, compact fat or settled product, with no redness, no downtime. Focused ultrasound also reduces fat, so Dr Sin Yong does not use Ultherapy for lean faces; he uses the VF Lift – Vertical Facelift there.

Key facts
Ultherapy Prime
Micro-focused ultrasound with visualisation from Merz Aesthetics; DeepSEE imaging, then points at 1.5, 3.0 or 4.5 mm, the deepest at the SMAS
US clearances
Ultherapy is cleared for lifting the brow, the neck and the tissue under the chin, and for lines of the décolletage
Time Freeze Laser LCLR®
A long-pulsed 755 nm and 1064 nm laser across the dermis at several depths, with a Sculpting mode for sagged fat and settled filler
Lean faces
Neither is planned; focused ultrasound reduces subcutaneous fat, and the VF Lift – Vertical Facelift is used instead
Recovery
Time Freeze, no redness and no downtime; Ultherapy, redness that settles, with tenderness, swelling or numbness for a time

Time Freeze Laser LCLR® vs Ultherapy: imaged points, or a field across the dermis

Time Freeze Laser LCLR® vs Ultherapy is first a question of where the heat goes. Ultherapy is micro-focused ultrasound with visualisation, made by Merz Aesthetics. Before each line, DeepSEE imaging shows the dermis, fat, SMAS and bone beneath the transducer. The beam then converges at 1.5, 3.0 or 4.5 mm, raising a tiny volume of tissue to roughly 60 to 70 °C. The 4.5 mm depth reaches the SMAS and, in the neck, the platysma. Dr Sin Yong performs Ultherapy on the Prime platform himself.

Time Freeze Laser LCLR® is his protocol on a long-pulsed 755 nm and 1064 nm laser platform. It works in the dermis and the superficial fat beneath it, as a field rather than at points, directing energy at the superficial, mid and deep dermis in one visit. Collagen contracts during treatment and remodels afterwards.

They reach different layers. Ultherapy reaches the SMAS; Time Freeze does not. Time Freeze has a Sculpting mode for sagged fat and settled product; Ultherapy has none aimed at those.

Lean, puffy or normal fullness: the rule that decides

Focused ultrasound also reduces subcutaneous fat where it is delivered. In a face with fat to spare that can be acceptable; in a lean face the same effect reads as hollowing. For that reason Dr Sin Yong does not use Ultherapy or HIFU on lean faces. There he uses the VF Lift – Vertical Facelift, whose Restore mode is used to stimulate the patient's own fat in hollow areas.

Time Freeze answers the opposite face. Its Sculpting mode is reserved for zones that are too full: sagged fat along the jowl and nasolabial area, the double chin, fat that blurs the jawline, and filler or biostimulator that has settled where it was not wanted, mapped with ultrasound first. The lean parts of the face are left alone.

Ultherapy keeps a clear role where descent is structural, at the SMAS, in a face with normal fullness. His summary: lean faces, VF Lift; puffy or heavier faces, Time Freeze; structural descent with normal fullness, Ultherapy has a place. Nothing is injected with any of these, so there is no filler to overfill, migrate or form lumps.

“Ultrasound can tighten a face with fat to spare. When a face is lean I use the VF Lift, and when it is heavy I use Time Freeze. The face decides.”

Which suits which face

Dr Sin Yong performs each of these, so the table is about what the energy does, not which is ranked higher.

What each cannot do

After Ultherapy, redness usually settles as the skin cools; tenderness along the lines, mild swelling, small bruises or a patch of numbness can persist for a time. Rarely there is a surface burn or temporary weakness where heat reaches a nerve branch. With Time Freeze there is no redness and no downtime; less often tenderness where excess was targeted, or temporary darkening, and rarely a burn, blister or more fat loss than intended. More detail is on Ultherapy side effects.

Brow, jowls and neck: where each has a role

Brow: Ultherapy holds a US clearance for lifting the brow, using the superficial depths around the eyes within the bony rim. Time Freeze's periorbital setting is directed at lax upper-lid skin and brow heaviness, with conservative settings. True ptosis of the eyelid muscle is referred.

Jowls: a jowl can be SMAS laxity, sagged fat, or both. SMAS laxity in a face with fat to spare can suit Ultherapy. A jowl heavy with compact fat is what the Sculpting mode of Time Freeze is directed at; jowls and lower-face sagging sets out how they are told apart.

Neck: Ultherapy reaches the platysma at 4.5 mm and holds a neck clearance. For the neck, Dr Sin Yong starts most plans with the VF Lift – Vertical Facelift and Time Freeze Laser LCLR®, because neck skin is thin, and keeps the thyroid out of the field with any energy.

Sequencing, and how Dr Sin Yong chooses

Plans sometimes sequence Ultherapy with one of his protocols when a face has more than one finding, decided at review rather than stacked on the same day. Time Freeze Laser LCLR® is most often paired with the VF Lift – Vertical Facelift, and the two can be used together on the same face.

He examines the face at rest and in animation, pinches and lifts the skin for recoil, palpates fat thickness over each zone, and maps earlier filler with ultrasound. If Ultherapy is not the right mechanism, he says so. Fees follow the plan; see how fees are quoted.

Frequently Asked Questions

It is not a ranking, and Dr Sin Yong performs both. Ultherapy places imaged focused ultrasound points down to the SMAS and suits structural descent in a face with normal fullness. Time Freeze Laser LCLR® heats the dermis at several depths and adds a Sculpting mode for sagged fat and settled product. Which suits depends on how much fat each zone has.

A heavy lower face is usually laxity over sagged fat or settled product, not laxity alone. Ultherapy does not reduce a full fat pad on purpose, and tightening over fullness can look heavier. Time Freeze Laser LCLR® tightens the dermis while its Sculpting mode targets the compact fat along the jowl and under the chin. The examination confirms what the heaviness is.

Time Freeze Laser LCLR® is not a treatment for hollowing; its Sculpting mode takes away excess. Loss of subcutaneous fat is a recognised side effect of focused ultrasound. Where hollowing is established, the discussion is about restoring support, and the VF Lift – Vertical Facelift, whose Restore mode is used to stimulate your own fat, is what Dr Sin Yong reaches for.

Ultherapy is felt as heat and, at the 4.5 mm depth, a brief deep ache along each line, most noticeable over bone and along the jaw; analgesia is discussed and provided according to the areas. With Time Freeze Laser LCLR® the treated zones feel warm, and the warmth fades soon after. Tolerance varies between people with either treatment.

Ultherapy reaches the platysma at 4.5 mm and holds a US clearance for lifting the neck and under-chin. Neck skin is thin, so Dr Sin Yong starts most neck plans with the VF Lift – Vertical Facelift and Time Freeze Laser LCLR®, which is used for loose, crepey neck skin. Vertical bands are muscle, and are a botulinum toxin question rather than an energy one.

Sometimes. When a face has structural descent in one area and heaviness in another, a plan can sequence Ultherapy with one of Dr Sin Yong's protocols, with the order and spacing decided at review rather than stacked on one day. Time Freeze Laser LCLR® is most often combined with the VF Lift – Vertical Facelift. What suits you is decided at assessment.
