Uneven face (facial asymmetry):
causes and non-surgical options

One side fuller than the other after filler: see uneven filler.

An uneven forehead contour, where one side sits flatter, is assessed under forehead filler.

Where one masseter is larger than the other, the dose can be weighted to that side; see jaw slimming injections Singapore.

A chin that sits off-centre is one of the commoner asymmetries; the guide to chin fillers Singapore covers how small amounts are placed on the shorter side.

Some facial asymmetry is normal, because no face is an exact mirror image. It becomes a clinical question when it is marked, has changed, is progressing or comes with pain, numbness, weakness or a bite change. The cause can sit in bone, muscle, fat, skin or habit, and each answers to a different approach. Botulinum toxin and filler can adjust soft-tissue differences, energy devices tighten skin but do not move bone, and bony or bite-related asymmetry is referred on.

LookalikesOften confused with a bone difference or uneven filler when one masseter is simply larger from habit: clench your teeth and a muscle cause firms up on one side, while bone and soft-tissue volume do not change. masseter bulk.

Is facial asymmetry normal?

Yes, a degree of asymmetry is normal. The two sides of almost every face differ slightly in the brows, eyes, cheeks, nose, lips and jaw, and research that maps faces in three dimensions grades asymmetry along a spectrum, from relative symmetry through moderate to severe. Small differences are part of how a face looks, and many are noticed only when someone points them out or a photograph is taken at an angle.

Asymmetry becomes a clinical question in a few situations. It is marked or distressing, it has changed over time, it appears after an injury, dental work or a procedure, or it comes with symptoms such as pain, numbness, weakness, difficulty chewing or a change in how the teeth meet. Those cases are examined rather than simply treated cosmetically.

The aim of any non-surgical plan is balance, not a mirror match. Correcting every difference is neither possible nor desirable, and treatment that chases exact symmetry can create new asymmetry.

Weight change, ageing and dental work can all make an existing difference more visible, because the volume and support that once disguised it change. A face that looked even earlier in life may not look even later, and that does not mean something has gone wrong.

What causes facial asymmetry in bone, muscle, fat and habit?

Five layers can contribute, and a face usually has a blend. Bone sets the frame: the jaw, chin, cheekbones and nasal bones can differ in width, length or position between sides. Muscle contributes through bulk, and in the jaw the masseter enlarges with use, so one side that clenches or chews harder can look wider.

Fat and soft tissue contribute through volume, which can be unequal between the cheeks, temples or under the eyes and can shift with weight change and ageing. Skin contributes through laxity, which may be looser on one side. And habit contributes in several ways: clenching or grinding that loads one side more than the other, a strong preference for chewing on one side, and sleeping or posture habits.

The distinction matters because the layers answer to different treatments. Only muscle responds to muscle treatment, soft-tissue volume responds to placed product, laxity responds to energy, and bone does not respond to any of them. The causes of a wide lower face are set out on the square jaw and masseter page, and the same logic applies to one-sided differences.

“Clench your teeth with your fingers on the jaw angle — if the width hardens under your fingers it's muscle, and only muscle answers to muscle treatment.”

How is facial asymmetry assessed, and what does that decide?

Assessment is an examination, and it comes before any product or device is named. Dr Sin Yong examines the face at rest and in movement, from the front and the side, because a face that looks even when still can differ on smiling, clenching or raising the brows. History covers when the difference appeared, any injury, dental work or earlier treatment, and symptoms such as jaw pain or headaches.

The clench test separates muscle from bone and fat. With fingers flat over the jaw angles, a masseter-driven width hardens and bulges when the teeth are clenched, while bony width is fixed and fat is soft. Where clenching or grinding is suspected, a dental review is part of the conversation, because the habit that built the muscle can also wear the teeth.

The assessment maps structure, skin quality, laxity and descent layer by layer, as described on the Private Aesthetics Analysis page. The result is a plan that names which layer is responsible for which difference, what can be adjusted non-surgically, what cannot, and whether referral is the honest answer.

Photographs are taken for the medical record, not for advertising, from set angles so that like is compared with like at review. Staging lets balance be judged as swelling settles, before the next step is decided.

What can injectables and energy devices do, and what can they not?

Botulinum toxin works on muscle and nothing else. Where one masseter is bulkier, toxin can reduce that muscle's activity over time and so narrow the lower face on that side, but it does not change bone or fat. Dosing is planned to preserve comfortable chewing, and the jaw slimming page covers the details. Less commonly the product spreads to a neighbouring muscle and the smile looks uneven, or an untreated part of the muscle bulges on clenching, which is why a review is part of the plan.

Filler adds volume where tissue is lacking. A cheek that has deflated more on one side can be supported with placed product, and a chin that is retruded can be projected, but filler replaces lost volume and does not lift loose tissue. Cheek filler and chin enhancement are matched to the finding, and the chin is not the tool for a jaw that sits off-centre or a bite that does not meet. Filler can itself cause asymmetry, lumps or an overfilled look, so volumes are conservative.

Energy devices act on skin and the tissue just beneath it. Where one side is looser, radiofrequency such as VF Lift – Vertical Facelift, built on Volnewmer monopolar radiofrequency, or focused ultrasound can be directed at the laxity, and the plan can be weighted by zone. They tighten and lift soft tissue; they cannot change bone, move a jaw or refill a hollow.

Who is it suited to, and who is referred on?

A non-surgical plan suits soft-tissue differences in a face with a stable bite and healthy skin: unequal muscle bulk, unequal volume in the cheeks or chin, or unequal laxity. It suits people who accept that the goal is a better balance and not exact symmetry, and who are prepared for a staged plan with review.

Some findings are referred. A marked skeletal discrepancy, a jaw that deviates, or a bite that does not meet goes to orthodontic or maxillofacial assessment. A deviated nose, or marked nasal asymmetry, goes to a plastic surgery specialist. Asymmetry after trauma, or that is progressing, goes for medical assessment first, and surgery is never replaced by a product that cannot reach the problem.

Pregnancy and breastfeeding, active infection or inflamed acne at the treatment site, and certain neuromuscular conditions are reasons to wait or to choose differently, and are declared at the consultation. Sudden weakness or drooping on one side of the face, new numbness, slurred speech or weakness of an arm or leg is an emergency: call 995 now, and do not travel to a clinic first.

What are the risks, and when should you call?

Every treatment option carries its own risks, explained beforehand. Botulinum toxin can cause bruising, headache, weaker chewing for a period, an uneven smile or a paradoxical bulge, and it is unsuitable in pregnancy and with neuromuscular conditions. Filler can cause swelling, bruising, lumps, asymmetry, product that moves, and, rarely, a blocked blood vessel.

A change in vision or eye pain after filler: call 995 now. Skin that turns white, dusky or mottled after filler, or pain that is severe or getting worse: urgent assessment by the doctor who injected you, or at A&E, now. A smile that looks uneven after botulinum toxin, or a lump, should be reviewed rather than watched. Increasing swelling, redness or heat needs a doctor the same day; if no doctor can see you today, A&E. The page on complication care sets out what to do.

Response varies between people, and some residual asymmetry is expected. A plan that promises exact symmetry should be treated with caution.

What determines the fee for treating facial asymmetry?

Singapore rules prevent clinics advertising prices, so factors are given here, not figures. The fee depends on what the assessment finds, which layers are involved, the areas treated, the products or devices chosen, whether treatment is combined, and how the plan is staged with review.

Because the same asymmetry can have different causes, a figure quoted before examination would not be meaningful. Dr Sin Yong gives a written quote at consultation, after assessment, and the consultation also decides whether treatment, referral or no treatment is the right step. How quotes work is explained on the how we quote page.

Frequently Asked Questions

When the unevenness comes with a change in your bite, jaw pain, clicking or difficulty opening the mouth, or when it seems to follow the jaw bone rather than soft tissue. Those patterns point to the teeth or the jaw joint and are assessed by a dentist or orthodontist before any cosmetic treatment is considered.
Sometimes. When one side of the smile pulls higher or harder because a muscle is more active, small doses of botulinum toxin on the stronger side can balance it. A smile that changed suddenly, or comes with weakness elsewhere in the face, needs medical assessment first, because nerve problems are treated differently. New weakness of the face, an arm or a leg, or slurred speech: call 995 now, and do not travel to a clinic first.

Yes, a degree of asymmetry is normal. Many faces differ slightly between sides in the brows, eyes, cheeks, nose and jaw. It is examined when it is marked, has changed, follows an injury or comes with pain, numbness, weakness or a bite change.

The fee depends on what the assessment finds, which layers are involved, the areas, the products or devices chosen and how the plan is staged. Singapore rules prevent clinics advertising prices, so Dr Sin Yong gives a written quote at consultation, after examination.

It is worth considering when the difference is soft-tissue in origin, bothers you, and you accept that the goal is better balance rather than an exact match. If the cause is bone or the bite, a non-surgical plan is the wrong tool.

No duration can be promised. Botulinum toxin and filler are temporary and are reviewed over time, while the underlying habit, ageing and weight change continue. Dr Sin Yong sets review timing at consultation and adjusts it as your face responds.

Treatment adjusts balance and cannot promise symmetry. Botulinum toxin can cause weaker chewing or an uneven smile, and filler can cause swelling, lumps, asymmetry or, rarely, a blocked blood vessel. Bone and bite causes are not treatable non-surgically.

Only if the unevenness is muscle. A bulkier masseter on one side can be reduced with botulinum toxin, but bone width and fat do not respond. The clench test and examination decide which applies before any injection.

When it is sudden, progressive, follows injury, or comes with weakness, drooping, numbness, pain or a bite change. Sudden one-sided weakness, slurred speech or arm weakness is an emergency: call 995 now, and do not travel to a clinic first.
