Bipolar radiofrequency is a way of delivering radiofrequency energy in which the current flows between two electrodes that sit in the same handpiece or needle tip. Because the current only travels between those electrodes, the heat stays in a defined zone close to the surface or around the needle tips. Most RF microneedling devices, including Secret RF, are bipolar.
With bipolar delivery, the doctor controls where the heat lands by choosing the needle depth and the spacing of the electrodes; the energy does not travel far beyond them (Chandra, J Cutan Aesthet Surg 2024). Monopolar devices such as Thermage FLX and Volnewmer send the current from the treatment tip to a return pad on the body, which heats a broader volume of tissue and is used for tightening rather than for working at a set needle depth. Frequency changes the picture too: at 2 MHz the heat tends to stay closer to the needle, while lower frequencies spread it further. The device-by-device figures, taken from each manufacturer's US FDA filing, are on RF microneedling devices compared.
Most RF microneedling devices are bipolar. A few, such as Potenza, offer both modes in different tips (FDA K192545).
Neither is safer in itself. They heat tissue in different patterns and suit different problems; settings, skin type and the doctor's assessment matter more than the mode.
