Acne removal surgery Singapore searches and the phrase acne scar surgery cover a short list of genuinely surgical procedures and a long list that are not. Punch excision, punch elevation and punch grafting are surgical and are referred to a plastic surgery specialist. subcision is a minor procedure done in clinic under local anaesthetic with no incision. Lasers, RF microneedling and TCA CROSS are not surgery at all. Dr Sin Yong, an aesthetic physician, performs the non-surgical work himself and refers the rest.

Short answerMost acne scars do not need surgery. Surgical revision suits an isolated pit too deep or wide for TCA CROSS or resurfacing, or a scar that has stretched, contracted or distorts an eyelid or lip; those go to a plastic surgery specialist. Everything else, which is most of what a scarred cheek needs, is done without cutting: release, support and resurfacing, as set out in the acne scar treatment Singapore guide.

Acne removal surgery Singapore: what is actually surgical

Surgery means cutting skin and closing it. For acne scars that covers a small group of techniques, all aimed at individual scars rather than a field. Punch excision removes a deep scar with a small circular blade and closes the wound with sutures, exchanging a pit for a fine line. Punch elevation cuts around the scar base and raises it level with the surrounding skin, fixing it there to heal. Punch grafting removes the scar and replaces it with a small skin graft. Formal scar revision, which excises a wide or stretched scar and re-closes it in layers under less tension or re-orients it, belongs to scars that have widened, contracted across a joint or pulled on an eyelid or lip, which is unusual for acne. All of these are surgical and all are performed by a plastic surgery specialist, or in some settings a dermatology specialist, after their own examination. Dr Sin Yong does not perform them. Where one is indicated he refers, and plans any resurfacing around it, because a revised scar is a new, immature scar and the texture around it still needs the usual work.

What is not surgery

Most of what people search for as acne scar surgery is not surgical. Fractional CO2 resurfacing with the S3 Resurfacing Lift vaporises microscopic columns of skin and leaves intact bridges between them; it is a laser procedure under local anaesthetic, not an operation. FSX Laser heats the dermis without removing the surface. RF microneedling on the Secret DUO passes fine insulated needles to a set depth and releases bipolar radiofrequency at the tip, with the surface spared (Syder 2023). TCA CROSS places a microdroplet of high-strength trichloroacetic acid into the floor of each ice-pick pit (Lee 2002). Radiesse is injected beneath a released scar. Intralesional injection softens a raised scar. None of these cuts skin or needs a stitch, and all of them are performed in clinic by Dr Sin Yong himself. The question of which to use for which scar is on the acne scar laser treatment and acne scar resurfacing pages.

Where subcision sits

subcision is the procedure that confuses the labels. Its full name is subcutaneous incisionless surgery (Orentreich 1995), so the word surgery is in it, but there is no incision: a fine needle or blunt cannula is passed through a small puncture beside the scar and swept beneath it to divide the fibrous band holding it down. It is done under local anaesthetic in clinic, takes one visit, and is followed by bruising and swelling that settle rather than by sutures and wound care. It is better described as a minor procedure. It is also the step that most often decides whether a scar plan works, because a tethered rolling scar shrugs off every laser pass until the band is released. Dr Sin Yong performs subcision himself as the first stage of 4D Scar Reconstruction.

Acne scar surgery: when a plastic surgery specialist is the right referral

Surgical revision earns its place where the problem is a scar’s width, shape or position, which no energy device or injectable changes. For acne, that usually means an isolated ice-pick or deep boxcar scar that is too deep or too wide for TCA CROSS and that resurfacing will not reach; a punch technique exchanges it for a fine surgical mark, and the texture around it is then treated with laser or RF. Less commonly, an acne scar has stretched into a band or sits where it distorts a free margin. Keloids are a separate caution: cutting a keloid out on its own frequently brings it back, often larger, so excision is reserved for selected keloids that have not responded to intralesional therapy, and is always followed by adjuvant treatment. When Dr Sin Yong finds any of these at assessment, the referral is written into the plan and the non-surgical stages are sequenced around it. The who treats acne scars in Singapore page sets out who does what in more detail.

What Dr Sin Yong performs himself

Everything non-surgical in the scar plan is performed by Dr Sin Yong personally on Orchard Road: the grading, subcision, Radiesse placement, TCA CROSS, every pass of the S3 Resurfacing Lift and FSX Laser, and RF microneedling on the Secret DUO. He is an aesthetic physician with an MSc in Aesthetic Medicine with Distinction from Queen Mary University of London and an MSc in Practical Dermatology from Cardiff University; he is not a registered specialist and this site does not describe him as one. The sequence he uses for mixed scarring is release, support, then resurface, because resurfacing remodels skin in whatever position it finds it, and a plan that leaves a tether in place or a volume deficit unsupported treats the wrong structure; the acne scar treatment Singapore guide sets out that sequence in full. What that visit looks like is on the what a scar visit on Orchard Road involves page.

Risks, recovery and the honest endpoint

Non-surgical scar work has its own recovery: bruising and swelling after subcision, a white frost then a small crust on each pit after TCA CROSS, redness, swelling and fine crusting after the S3 Resurfacing Lift, redness and pinpoint marks after RF microneedling. Less often there can be prolonged redness, pigment change (more likely in Fitzpatrick III to V skin and with sun exposure), infection, lumps from an injected product or a scar that responds less than hoped. Surgical revision carries sutures, wound care and a new scar that must mature before it can be judged, set out by the operating specialist. Neither route returns skin to an unmarked state; the realistic aim is a scar that is shallower, softer and less visible under normal light. How the two routes compare across all scar types is on scar removal surgery vs non-surgical options, and every acne and scar guide on this site is indexed on one page.

“Surgery exchanges one scar for another, ideally a finer one. For most acne scars the better exchange is release, support and resurfacing, without a blade.”

Dr Sin YongOn surgical and non-surgical scar work

Dr Sin Yong explains

I am a member of the Royal College of Surgeons of Edinburgh, and people sometimes assume that means I operate on scars. I do not. I am an aesthetic physician, and the scar work I do is the non-surgical kind: subcision through a needle puncture, acid into ice-pick pits, Radiesse under released scars, and the lasers. That covers most of what a scarred face needs. When I find a scar that genuinely needs a punch excision or a graft, I refer it to a plastic surgery specialist and plan my work around theirs, because that is the right person for that scar. Knowing what is surgery and what is not is part of being honest about what I can do.

Frequently Asked Questions

Rarely. Surgical revision suits an isolated pit too deep or wide for TCA CROSS or resurfacing, or a scar that has stretched, contracted or distorts an eyelid or lip. Most acne scars are treated without cutting: subcision for tethers, TCA CROSS for ice-pick pits, Radiesse for lost volume and fractional CO2 or non-ablative laser for the surface.

Its full name is subcutaneous incisionless surgery, but there is no incision: a needle or blunt cannula passes through a small puncture and divides the band beneath the scar, under local anaesthetic, with no stitches. It is better described as a minor procedure done in clinic. Bruising and swelling follow and settle; Dr Sin Yong performs it himself.

No. Punch excision, punch elevation and punch grafting are surgical procedures performed by a plastic surgery specialist, or in some settings a dermatology specialist. Dr Sin Yong is an aesthetic physician; where a punch technique is indicated he refers, and plans any resurfacing around it.

No. Fractional CO2 resurfacing vaporises microscopic columns of skin and leaves intact skin between them; it is a laser procedure under local anaesthetic with redness, swelling and crusting while the surface heals, not an operation. The S3 Resurfacing Lift on the DEKA Tetra Pro is Dr Sin Yong's protocol for it.

Not on its own. Excision alone frequently brings a keloid back, often larger. First-line treatment is intralesional therapy, often with silicone, pressure or laser. Surgery is reserved for selected keloids that have not responded, is referred to a plastic surgery specialist, and is always followed by adjuvant treatment.

The plastic surgery specialist examines the scar and sets the procedure, costs, sutures and wound care. A revised scar is a new, immature scar, so Dr Sin Yong plans any resurfacing or RF work around it once it has settled, and the rest of the scar map is treated as planned.
