What to expect at a Profhilo visit with Dr Sin Yong begins with one question: is the concern skin quality or lost volume, because Profhilo adds no volume. If it fits, numbing cream can be applied, small deposits are placed through a fine needle at the Bio Aesthetic Points with extra placement where your face needs it, raised blebs appear at the points and flatten as the product spreads.

Before any Profhilo is placed there is an assessment, and it decides whether Profhilo is the right category. Profhilo is a stabilised hybrid complex of high- and low-molecular-weight hyaluronic acid, made by IBSA, that acts on the skin itself. It is directed at crepey, dull or thin skin, poor hydration and fine surface lines, including the neck and the backs of the hands. It is the wrong choice for a hollow cheek, a flat bridge, a receded chin, jowling or a loose jawline, and Dr Sin Yong says so when that is the finding.

Before the visit:
Declare any allergy to hyaluronic acid or previous reaction to injectables.
Tell the clinic about pregnancy, breastfeeding, cold sores and any autoimmune condition.
Mention blood thinners, aspirin, fish oil or supplements that increase bruising. Do not stop a prescribed medicine on your own.
Avoid fresh sun exposure and pause strong skincare actives as advised.
Bring photos, a list of previous injectables and records of earlier treatments.

How is Profhilo placed at the visit?
Numbing cream can be applied first. Profhilo is then placed through a fine needle as small, separate deposits, called boluses. Despite its relatively high hyaluronic acid content, the hybrid complex has a low viscosity, which is why a fine needle suits it and why each deposit diffuses through the tissue from where it sits.
The manufacturer specifies a defined set of Bio Aesthetic Points on each side, chosen for two reasons: each point sits away from the major facial vessels, and from each point the product spreads predictably. Dr Sin Yong treats those points as the safety framework rather than the whole plan. He keeps them, then adds placement according to the shape of the individual face: where the midface has deflated, where the jawline has softened, where the asymmetry sits. He also places it into scarred skin, which the standard protocol does not address. The anatomical rules for where an injectable may safely sit do not change; the map does. Most people feel a series of small pricks with some pressure as each deposit is placed. Comfort is checked as the points are completed, and you can ask to pause.

What happens straight after Profhilo, and what needs a call?
Small raised blebs appear at the injection points straight afterwards, because the product is placed as discrete deposits. That is the expected appearance of a correct deposit, not a complication. Mild redness is common, and bruising is more likely near the angle of the jaw and in front of the ear. These are phases, not a timetable:
Immediately after. Blebs at the points and mild redness.
The first evening. Avoid make-up over the points, heat, alcohol and exercise. Do not massage the blebs unless you have been told to; they flatten as the hyaluronic acid spreads on its own.
The following phase. Bruising, tenderness and transient swelling settle, and the skin looks ordinary once the blebs have dispersed.
As the skin settles. Changes in hydration and elasticity develop gradually rather than on the day.
What needs a call
Pain that is getting worse: urgent assessment by the doctor who injected you, or at A&E, now.
Skin that turns pale, dusky or mottled: the same, now.
Spreading redness or swelling that persists: a doctor the same day; if no doctor can see you today, A&E.
The review reads how the skin has responded once the material has integrated, rather than on the day. Any second placement is timed at review rather than booked in advance.
A bump that is still there once the rest of the skin has settled is scanned before anything is decided, because filler or another product placed earlier can sit in the same area. If the lump is the Profhilo itself, it is hyaluronic acid, and hyaluronidase dissolves hyaluronic acid. Before any planned dissolving, allergy history is checked and a skin test is done. A lump that is red, hot or tender needs a doctor the same day and is treated medically first.
The review is also where the next layer is considered. If the examination shows that the lateral face has flattened, Profhilo Structura, a separate IBSA product placed deeper in the superficial fat compartments, may be assessed. No. Profhilo Structura is a separate IBSA product with more hyaluronic acid, placed deeper, in the superficial fat compartments of the lateral face, at a different set of points. It is considered for early flattening of the lateral face rather than skin quality. If a clinic has quoted you for Profhilo without saying which product, ask which one is planned.
