You are suitable for Thermage when the problem is the skin envelope: mild to moderate laxity in a face, eyelid, neck or body area that has kept its volume, in skin that still recoils when pinched, with no implanted electronic device. It suits descent of the deeper layers, lost volume and heavy fat poorly, and those faces are directed to HIFU or VF Lift – Vertical Facelift instead.

Key facts
Suits
Mild to moderate laxity of the cheeks, jowls, jawline, neck, eyelids or body in skin that still recoils, where volume has been kept
Suits poorly
Marked descent of the deeper layers, hollowing or lost volume, heavy fat under the chin or jowl, or large skin excess
Does not suit
A pacemaker, implantable defibrillator or other implanted electronic device; pregnancy; infected, broken or inflamed skin in the area
Changes the plan
Metal implants near the area, recent filler or threads where the tip will pass, a heat-sensitive skin condition, slow healing, reduced sensation
Eyes
The eye tip suits crepey lid skin and mild hooding from skin laxity, not fat bags, brow descent or marked lid excess
Elsewhere
Deeper laxity is a question for HIFU or VF Lift – Vertical Facelift on Volnewmer; a lean face is planned for VF Lift, a puffy one for Time Freeze Laser LCLR®

In brief

Suitability for Thermage depends on whether the skin envelope or the deeper structure has changed. Early laxity of the cheeks, jawline, neck, eyelids or body, with volume kept, suits it; marked descent, hollowing or heavy fat suits it poorly. A pacemaker or other implanted electronic device rules it out, and deeper laxity is a question for HIFU or the VF Lift – Vertical Facelift.

Who is suitable for Thermage, and what the assessment looks for

Thermage FLX is monopolar radiofrequency at 6.78 MHz with cryogen cooling of the surface. It heats the deep dermis and the fibrous septae beneath it as a volume, prompting collagen contraction and remodelling in the skin envelope. It does not reach the SMAS, add volume, reduce fat or remove skin. You qualify when the skin itself has loosened and little else has changed. The Thermage FLX page explains the device; this page answers whether your face, eyes or body qualify.

Dr Sin Yong examines the area first and separates three things that look alike in the mirror. Laxity of the skin envelope is a surface that no longer recoils cleanly when pinched but sits over tissue that is still where it belongs. Descent is tissue that has slid, so a jowl forms whatever the surface looks like. Volume loss is a hollow that reads as a sag because support has gone. Thermage acts on the first only. He also takes the history that governs monopolar radiofrequency: implanted electronic devices, metal near the field, recent filler or threads in the plane the tip will pass, skin conditions that flare with heat, and whether you can feel heat normally, because energy is chosen partly from what you report during each pulse.

Early laxity versus descent: the distinction that decides it

Early laxity is the Thermage indication. The skin over the cheek or along the jawline has lost some of its spring, there is a softness rather than a fold, and when you tuck your chin the skin creases without a pocket of tissue hanging over the jaw. Here bulk heating of the dermis is a sensible thing to do, and the single-use face tip is chosen for the cheeks, jowls, jawline and neck.

Descent is different. When the jowl is a pocket that hangs over the jawline, when the midface has slid so that the fold beside the nose has deepened, or when the whole lower face hangs because the fascia has loosened, the change is in a layer Thermage does not reach. Those faces are directed to a mechanism that works deeper: focused ultrasound, which heats discrete points down to the SMAS, or VF Lift – Vertical Facelift, Dr Sin Yong's protocol on Volnewmer monopolar radiofrequency, applied along vertical vectors and calibrated per zone.

What the examination findsThermage? What is weighed instead or alongside

Crepey or loose neck skin with little fat beneathYes, face tip on the neckNeck assessed separately from the face
Jowl hanging over the jawline, fold deepened by a slid midfacePoorlyHIFU or VF Lift – Vertical Facelift
Lean or hollow face with early laxityCaution; heat into thin tissue can hollow furtherVF Lift – Vertical Facelift planned for the lean face
Puffy, heavy face or full jowl from fatNoTime Freeze Laser LCLR® to take away the excess first
Fullness under the chin from fat or a short chinNoAssessed as a double chin question
Marked skin excessNoSurgical opinion

Hollowing after any energy-based tightening is a recognised side effect, and a lean face has the least tissue to spare. That is why Dr Sin Yong plans VF Lift – Vertical Facelift for the lean face and Time Freeze Laser LCLR® for the puffy one, and reserves Thermage for the skin envelope where volume has been kept. The Thermage vs HIFU vs Volnewmer article sets the three mechanisms side by side.

“Thermage acts on the skin envelope and nothing else. If the examination finds descent, fat or a hollow, heating the skin over it is not a plan, and I will not call it one.”

Dr Sin YongOn who is suitable for Thermage

Thermage for the eyes and the body: who each tip suits

The eye tip is small and delivers lower energy to the thin skin of the upper and lower lids, with protective shields inserted under the lids after anaesthetic eye drops. It suits crepey eyelid skin and mild hooding that comes from the skin itself, with the brow in a reasonable position. It does not suit hooding that comes mainly from a descended brow, because the brow is the problem and the options differ, as set out under droopy and hooded eyelids; it does not change a lower-lid fat bag, because heat on the skin does nothing to fat, and eye bags are assessed on their own terms; and marked lid excess is a surgical question. The Thermage for the eyes page covers comfort and swelling.

The body tip is used for loose skin across a large surface: the abdomen after pregnancy or weight change, the inner arms and the thighs. It suits a surface that has slackened over a stable weight, where the skin still has some recoil. It suits fat as the main concern poorly, and where the examination finds a fat layer rather than a loose envelope the 4K Body Lift or the body protocols of Time Freeze Laser LCLR® are discussed instead. Large skin excess after major weight loss is beyond what any energy device reaches, and the honest answer there is a surgical opinion; the loose skin after weight loss page explains how that line is drawn.

Who should not have Thermage, and who should wait

One exclusion is firm. Monopolar radiofrequency passes an electrical current from the tip through the body to a return pad, so Thermage is not used in anyone with a cardiac pacemaker, implantable defibrillator or other implanted electronic device. It is postponed during pregnancy, where its safety has not been established, and over skin that is infected, broken or inflamed, including an active cold sore or a flare of eczema in the treatment area.

Other points change the plan rather than rule treatment out. Metal implants or plates near the area are noted, because current concentrates around metal. Recent filler or threads where the tip will pass are mapped, with ultrasound where the history is unclear, so that heat is not placed over material that is still settling. A skin condition that flares with heat, slow wound healing, and reduced sensation in the area are each weighed, the last because energy is set partly from what you feel during each pulse. Skin type is not a limiting factor, because radiofrequency is not absorbed by melanin.

Who should not have this treatment

The Thermage side effects page sets out what can go wrong and how often, and the Thermage aftercare page what recovery looks like and when to call.

What decides the plan, if you are suitable

Fees are not advertised, and a figure before examination would not describe your skin. What shapes the plan is which tip the area needs and how many pulses it carries, how many areas are combined in one visit, such as face and eyes or face and neck, how much laxity there is against how much volume has been kept, recent filler, threads or metal near the area, and whether another protocol is combined and how it is staged. The how we quote page explains what the written plan contains.

If you are suitable, the skin is cleansed and marked, a return pad is placed, and energy is given in several lower-energy passes guided by your report of heat, with cooling and vibration protecting the surface. Dr Sin Yong performs it personally with a genuine single-use tip. If you are not, the plan says which mechanism fits instead; the sagging face treatment page sets out the alternatives, and the Volnewmer vs Thermage comparison explains when the other monopolar platform is chosen.

Frequently Asked Questions

It depends on whether the jowl is skin softness or tissue that has slid. A softness along the jawline with no pocket hanging over the jaw, in a face that has kept its volume, suits Thermage. A jowl that hangs over the jawline is descent in a layer Thermage does not reach, and that face is directed to focused ultrasound or VF Lift – Vertical Facelift. Dr Sin Yong separates the two by examination.

Yes, when the problem is the skin itself: crepey eyelid skin or mild hooding from skin laxity, with the brow in a reasonable position. The small eye tip is used at lower energy with protective shields placed under the lids after anaesthetic drops. It does not suit hooding from a descended brow, a lower-lid fat bag, or marked lid excess, which are a brow, fat and surgical question respectively. Contact lenses are removed first.

Anyone whose laxity sits in the deeper support layer rather than the skin envelope: a hanging jowl, a slid midface, or a lower face that hangs because the fascia has loosened. HIFU heats discrete points down to the SMAS; VF Lift – Vertical Facelift heats the deeper layers on Volnewmer along vertical vectors. A lean face is planned for VF Lift, and a puffy face for Time Freeze Laser LCLR® first.

Not with a pacemaker, implantable defibrillator or any other implanted electronic device, because monopolar radiofrequency passes a current through the body to a return pad. Metal implants or plates near the area do not rule treatment out but change the plan, as current concentrates around metal. Recent filler or threads in the plane the tip will pass are mapped first, with ultrasound where the history is unclear.

Yes, when the finding is a slackened skin envelope over a stable weight, such as the abdomen after pregnancy or weight change, the inner arms or the thighs, and the skin still has some recoil. It suits fat as the main concern poorly, and where a fat layer is found the 4K Body Lift or the body protocols of Time Freeze Laser LCLR® are discussed instead. Large skin excess after major weight loss is a surgical question.
