Sylfirm X is a radiofrequency microneedling device made by Viol in Korea, used by a number of clinics in Singapore. Like every RF microneedling system it places fine needles into the dermis and releases heat from them; it adds a pulsed-wave mode to the usual continuous-wave mode. It is not used at this practice. Dr Sin Yong performs RF microneedling on Secret DUO.

Sylfirm X is one brand of radiofrequency microneedling device. RF microneedling is the treatment. Every system in this category does the same basic thing: fine needles are driven to a set depth in the dermis, radiofrequency current flows from them, and the heat creates small controlled zones of coagulation that the skin repairs with new collagen.

Sylfirm X is made by Viol in Korea. It was cleared by the US Food and Drug Administration in 2020 under the regulation for electrosurgical cutting and coagulation devices. Its filings describe a bipolar 2 MHz system with continuous-wave and pulsed-wave modes and adjustable needle depth.
Clinics in Singapore often offer it under its own name. That is where confusion starts. A patient can be offered Sylfirm X at one clinic and RF microneedling at another and believe they are choosing between two treatments. They are choosing between two devices that deliver the same category of treatment with different engineering. The engineering differences are real, and they are set out below. They are not the first question. The first question is whether RF microneedling of any brand is the right tool for the problem in front of you.
The pulsed-wave mode delivers radiofrequency in very short, repeated bursts rather than as one continuous flow. It is the feature Sylfirm X is most recognised for. In laboratory work on pulsed-type bipolar radiofrequency through non-insulated needles, the short pulses produced small zones of coagulation in the dermis together with thickening of the basement membrane, the layer between the epidermis and the dermis, and narrowing of small blood vessels.
That is why Sylfirm X is discussed for conditions where the basement membrane and small vessels are thought to matter: melasma, Riehl melanosis and rosacea-type redness. A Korean series of forty-four patients with refractory melasma or Riehl melanosis treated in pulsed mode reported clinical improvement in most, with no serious adverse events. It was a single-arm study without a control group, so it shows what the device can do rather than how it compares with other options.
The continuous-wave mode is conventional RF microneedling. It produces the larger coagulation columns used for atrophic acne scars, enlarged pores, texture and early laxity. A clinic offering Sylfirm X for scars is, in practical terms, offering RF microneedling for scars.
| Mode | How energy is delivered | Usually discussed for |
|---|---|---|
| Pulsed-wave | Very short repeated bursts through non-insulated needles | Melasma, Riehl melanosis, redness-prone skin, as a supporting step |
| Continuous-wave | Conventional RF microneedling heating | Atrophic acne scars, pores, texture, early laxity |
“The device is the instrument, not the treatment. I start from the scar, and the scar tells me whether needles are the right tool at all.”

Rolling and shallow boxcar acne scars, enlarged pores, uneven texture and early laxity are the problems for which RF microneedling has the strongest support, whatever the brand. Each involves dermal collagen that has been damaged or slowly lost, and controlled heat at a chosen depth is a reasonable way to prompt remodelling while sparing the surface. In Fitzpatrick III to V skin, delivering heat below the epidermis lowers, without removing, the risk of post-inflammatory darkening.
Melasma deserves its own line. It is a chronic condition with hormonal, vascular and ultraviolet drivers. Any energy-based treatment, pulsed or not, is a conservative supporting step inside a plan built on sun protection and topical therapy.
Sylfirm X is not used here. Dr Sin Yong performs RF microneedling personally on Secret DUO, made by ILOODA in Korea. The two platforms differ in three practical ways, and Secret RF on Secret DUO is the platform Dr Sin Yong prefers and uses.
| Feature | Sylfirm X | Secret DUO (used here) |
|---|---|---|
| Needles | Non-insulated; energy released along the exposed shaft | Insulated and non-insulated cartridges |
| Modes | Pulsed-wave and continuous-wave | Continuous RF microneedling; no pulsed-wave mode in the same form |
| Also on the platform | Dedicated RF microneedling device | A 1540 nm non-ablative fractional laser |
| Where each has an edge | Pulsed mode studied for refractory melasma and redness-prone skin | Insulated needles in Fitzpatrick IV to VI skin; laser in the same visit |
Insulated needles release energy only at the tip, so the heat bypasses the pigmented epidermis. They are generally Dr Sin Yong's choice in darker skin types; non-insulated needles suit broad, shallow textural work. Where a pulsed delivery is thought to suit a pigment- or redness-adjacent concern, that is a point in Sylfirm X's favour, and it is said plainly.
Dr Sin Yong was appointed by ILOODA to share his Secret DUO microneedling expertise with other doctors. That is disclosed here because it is relevant to reading a comparison written by him.
Whichever device is used, an RF microneedling visit follows the same broad shape. The skin is cleansed, a numbing cream is applied, and the needles are passed over each zone at the depth and energy chosen for it. Heat is felt as warmth and prickling. Comfort varies between people and between areas, and it is discussed beforehand.
Afterwards the skin is usually red and warm, sometimes with mild swelling and fine pinpoint marks where the needles entered. How visible that is depends on the depth, the energy, the needle type and your skin. Non-insulated needles, which release heat along the shaft, tend to produce more surface reaction than insulated needles, which is one reason needle choice is matched to skin type.
The RF microneedling aftercare page sets out the instructions given after treatment here.
Compare the plan, not the machine. A brand name tells you which device is in the room. It does not tell you what your scars are, how deep they sit, which needle type will be used, at what depth and energy, or whether anything should be done before the needles go in.
What decides the plan here:
If you were offered Sylfirm X for melasma or redness, the useful first step is a diagnosis. Melasma, sun spots, post-inflammatory darkening and red acne marks can sit side by side on one face, and each needs a different tool. Pigment is examined for depth and cause, redness is checked to see whether it is blood in vessels or inflammation, and only then is an energy treatment chosen. A device chosen before that examination is a guess.
If you have already had Sylfirm X elsewhere, bring the dates, the mode used if you know it and how your skin responded. Fees follow the plan and are set out in writing at consultation; how fees are quoted explains what the quote contains.
Every treatment is performed by Dr Sin Yong personally. Singapore Medical Council registered · invited to share his clinical expertise internationally for CLASSYS, DEKA, Fotona, Hironic and Ilooda.

Dr Sin Yong prefers Secret RF, which he performs personally on Secret DUO, choosing between insulated and non-insulated cartridges and setting depth and energy zone by zone after an in-person assessment. The 1540 nm fractional laser on the same platform can be used in the same visit when the assessment calls for it.
Yes, in category. Sylfirm X is a brand of RF microneedling device. Its continuous-wave mode behaves like conventional RF microneedling; its pulsed-wave mode delivers very short bursts studied for pigment and redness-prone skin. Comparing it with RF microneedling is comparing one device with the treatment it delivers, not two different treatments.
Its pulsed-wave mode has been studied for refractory melasma and Riehl melanosis in a single-arm series with encouraging findings, but without a control group. Melasma is a chronic condition. Any energy treatment is a conservative supporting step within a plan built on sun protection and topical therapy, and the pattern that worsens melasma is heat delivered too aggressively.
Its continuous-wave mode is conventional RF microneedling, which suits rolling and shallow boxcar scars. Tethered scars usually need subcision first and deep ice-pick scars are better matched to TCA CROSS, whichever device is used. The scar type decides the sequence, so the scars are mapped before any needling is planned.
RF microneedling is one of the more workable options in Fitzpatrick III to V skin because heat is delivered below the surface. Risk is never zero with any device: redness, pinpoint marks and post-inflammatory darkening can occur. Needle choice, conservative settings and sun protection afterwards matter more than the brand on the handpiece.
Both are Korean bipolar RF microneedling systems. Sylfirm X uses non-insulated needles with pulsed-wave and continuous-wave modes. Secret RF on the Secret DUO platform offers insulated and non-insulated cartridges and adds a 1540 nm non-ablative fractional laser. They differ in engineering, not in the category of treatment, and head-to-head trials were not found.
