Am I suitable for Ultherapy? Who it suits, who should wait, and who is directed elsewhere

Medically reviewed by Dr Sin Yong · Last reviewed · 7 min read · Jump to questions

Where the looseness is in the skin envelope rather than the SMAS, Thermage Singapore may fit better; this is decided at assessment.

If imaging is not needed for your face, cartridge-based HIFU treatment on the Ultraformer MPT is weighed at the same assessment.

You are likely to be suitable for Ultherapy when the main change is mild to moderate laxity of the brow, jawline, under the chin or neck, in a face that has kept its volume. A lean or hollow face, heaviness that is fat, and skin that hangs in folds are directed elsewhere. Dr Sin Yong decides suitability at an in-person assessment on Orchard Road.

Short answerUltherapy Prime images the tissue, then heats small points at 1.5, 3.0 or 4.5 mm. It tightens tissue that is lax; it cannot add volume, remove fat or remove skin. Dr Sin Yong does not plan it for a lean face, where the VF Lift – Vertical Facelift is used instead. This page sits under Ultherapy in Singapore.

Who is suitable for Ultherapy, and why the tissue decides

Ultherapy suits tissue that has loosened but not emptied. Ultherapy Prime is micro-focused ultrasound with visualisation, made by Merz Aesthetics: before each line the DeepSEE image shows the dermis, fat, SMAS and bone beneath the transducer, and the line then places discrete heat points at 1.5, 3.0 or 4.5 mm. The 4.5 mm depth reaches the SMAS, the fibrous layer that supports the lower face, and in the neck the platysma. The heated points contract and are repaired with new collagen, so the mechanism is tightening of what is already there.

That is why the zones it suits are specific. The brow and forehead are treated at shallower depths, because there is little SMAS above the brow. The jawline and lower face take 4.5 mm where the image shows enough depth, with 3.0 mm above. Under the chin and the neck are treated at the platysma where the tissue allows. The skin around the eyes and the décolletage take the superficial depth alone, kept within the bony orbital rim.

Age is not the deciding factor; the tissue is. A useful self-check before you come in: lift the skin gently at the jaw in a mirror. If the line improves, laxity is part of the picture. If the cheek above looks hollow, volume is part of it, and ultrasound does not add volume. Skin that hangs in folds is a surgical question.

Lean face, full face or heavy lower face: where Ultherapy fits

How much fat a face has to spare is one of the first findings that decides suitability. Focused ultrasound places heat in a layer; placed into thin fat rather than the SMAS, or delivered across a face with little fat to lose, it can cause volume loss that reads as hollowing. For that reason Dr Sin Yong does not plan Ultherapy or other HIFU for a lean face. A heavy lower face is the opposite problem, and it is not laxity either.

What the examination findsUltherapy? What is weighed instead or alongside
Laxity of the jaw, neck or brow in a face with normal fullnessYes, planned zone by zone from the imageThermage FLX where the skin itself is also lax
Laxity in a lean face, with or without hollowingNoThe VF Lift, with Lift for loose tissue and Restore for hollow areas
A heavy lower face from fat or settled fillerNo; the heaviness is not laxityTime Freeze Laser LCLR® and its Sculpting mode, or the VF Lift Sculpt mode
Laxity with a hollow temple or cheekFor the laxity, not the hollowFiller or a biostimulator for structure, staged with the lift
Tissue that has slid, with skin that still has elasticitySometimesThe Bliss Lift thread lift, which repositions rather than tightens
Skin hanging in folds, heavy jowlsNoReferral to a plastic surgery specialist

Many faces carry two of these patterns at once, so the plan may sequence Ultherapy with another mechanism rather than choose one. Dr Sin Yong performs Ultherapy Prime, the Ultraformer MPT, Thermage FLX, the VF Lift and the Bliss Lift himself, so the recommendation follows the anatomy rather than the device in the room.

“Ultherapy can tighten tissue that has loosened. It cannot put back tissue that has gone, so a lean face is not a face I plan it for.”

Dr Sin YongOn who suits Ultherapy

Who should not have Ultherapy, and who should wait

Some findings mean waiting, some change the plan, and some point to a different route altogether.

Reasons to wait

Reasons the plan changes

Reasons it is the wrong tool

A very thin face or hollow cheeks, heaviness that is fat rather than loose tissue, and an expectation of the change a surgical facelift gives. Ultherapy tightens; it does not add volume, remove fat, resurface the skin or remove skin excess. Nerve irritation along the jaw or above the brow and fat atrophy are the risks most specific to focused ultrasound, and both are reasons why the depth and the zones are confirmed on the image rather than assumed. What is normal afterwards and what is not is on Ultherapy side effects.

What is checked at the assessment before Ultherapy is planned

Suitability is decided in person, and the assessment follows the same order each time.

  1. History. Any pacemaker, implant, metal, threads or fillers, with dates where known; all medicines and skin conditions; pregnancy or breastfeeding.
  2. Examination. The face at rest and in animation, and the skin pinched and lifted to gauge its recoil. This separates laxity, which Ultherapy treats, from volume loss and fat, which it does not.
  3. Zone by zone. The brow, jawline, under the chin, neck and any other zone are judged separately, because a face can be lax at the jaw and lean at the temple.
  4. Earlier product. Where filler is suspected in the treatment plane, ultrasound is used to see it before any heat is planned.
  5. On the day. The DeepSEE image confirms the depth for each zone before each line. Where it shows thinner tissue than expected, a shallower transducer is used or the zone is left out.

If Ultherapy is not the right mechanism, Dr Sin Yong says so; that is a common outcome of the appointment. You leave with a written plan naming the zones and depths, what is kept out of the field, the fee factors and the option of no treatment. The visit itself is walked through on what to expect at an Ultherapy visit, and what sets the fee on Ultherapy cost factors.

If Ultherapy is not suitable: the alternatives

Not being suitable is a finding, not a dead end. The plan points to whatever matches the layer that has changed.

Frequently Asked Questions

Age is not what decides suitability for Ultherapy; the tissue is. A younger face with early laxity along the jaw may suit it, and an older face that has mostly lost volume may not. Dr Sin Yong examines the face at rest and in animation, pinches and lifts the skin to gauge recoil, and separates laxity from volume loss and fat before deciding.

Usually not. Focused ultrasound placed in thin fat, or across a face with little fat to spare, can cause volume loss that reads as hollowing. Dr Sin Yong does not plan Ultherapy or other HIFU for a lean face; he uses the VF Lift – Vertical Facelift instead, with the Lift mode for loose tissue and the Restore mode for hollow areas.

Often, once they have been assessed. Recent filler or threads in the zone are timed around rather than heated over, and where filler is suspected in the treatment plane it is seen with ultrasound first. A heavy lower face from settled filler is not laxity, so it is usually a question for Time Freeze Laser LCLR® rather than Ultherapy.

Skin colour matters less for Ultherapy than the structure underneath, because the surface is not broken and the heat is placed in deeper layers. What decides suitability in Asian faces is the same as elsewhere: whether the change is laxity, how much fat the face has to spare, and which depths the image shows are reachable. The Ultherapy for Asian skin page covers this in detail.

Anyone whose main concern is hollowing, heaviness from fat or skin that hangs in folds, because Ultherapy tightens and does not add volume, remove fat or remove skin. Pregnancy, breastfeeding, open wounds, infection or inflamed acne in the area are reasons to wait. A pacemaker or other implanted electronic device is declared and assessed first, and marked skin excess is referred for a surgical opinion.
