Exosomes are nanoscale vesicles — tiny membrane parcels that cells release to carry signalling molecules to other cells. In aesthetics they are marketed as bottled regeneration. The honest physician's summary: genuinely interesting biology, early but growing clinical evidence, and a regulatory landscape where what's in the bottle varies enormously — which makes the source and the setting matter more than the word on the label.

What are exosomes and why is everyone talking about them?

Cells talk to each other by post: they package signalling molecules into membrane vesicles — exosomes — and release them to be picked up by neighbours, where the cargo changes behaviour. The aesthetic hypothesis is direct: harvest exosomes from regenerative cell cultures and deliver their pro-repair signals to ageing or healing skin. The systematic-review literature finds genuinely encouraging early signals — improvements in skin texture, healing support after procedures — alongside the honest caveats of a young field: small trials, varied products and protocols [1,2].

What does the evidence support — and what's still marketing?

Where data is most consistent: exosomes as an adjunct after energy-based treatments — applied to freshly lasered or microneedled skin, where reviews report faster settling and skin-quality gains [1]. Where marketing outruns evidence: exosomes as a standalone anti-ageing transformation, claims of 'stem cell results without stem cells', and the quiet blurring between very different products — a characterised human-cell-derived preparation and a plant-extract vesicle are not the same biology wearing the same name [2]. How exosomes are used in clinic — and the regulatory line

In Singapore and most jurisdictions, exosome products are used topically — typically applied immediately after fractional laser or microneedling, when channels in the skin allow penetration — as part of the recovery-and-result protocol rather than as an injected therapy. Do exosomes actually work for skin?+

Early systematic reviews report genuine promise, especially as topical support after energy treatments. The field is young: small studies, varied products. 'Promising adjunct' is the honest current verdict.

Are exosome treatments safe?+

Topical use after procedures has a reassuring reported profile in the review literature. The safety question is really a product question — source and characterisation vary enormously between brands.

Are exosomes injected?+

In most jurisdictions including Singapore, exosome products are used topically, not injected — injectable regenerative support belongs to established, regulated biologicals like polynucleotides.

Are plant exosomes the same as stem-cell exosomes?+

No — they share a name and a vesicle structure, not a biology. Human-cell-derived and plant-derived products carry entirely different cargo. Ask any clinic which they use and why.

Should I buy exosome skincare?+

On intact skin, penetration of large vesicles is questionable — the clinical logic favours post-procedure application when channels are open. As shelf skincare, the evidence is far thinner than the pricing.
