Facial overfilled syndrome: what the term means

Facial overfilled syndrome is the medical term for a face that looks heavy, rounded or altered in expression because dermal filler has been placed in excess or in the wrong layer, usually over repeated treatments. The everyday name is pillow face. It is most visible in a smile, and it can usually be reduced when the filler is hyaluronic acid.

Where the term comes from
The term appears in the peer-reviewed literature from 2020, when an anatomical study linked the heavy look of an overfilled mid-face in a smile to a transverse facial septum in the cheek (Cotofana, Dermatol Surg 2020). Later papers described it as underdiagnosed, caused by repeated incorrect filler placement and worse with expression and ageing (Lim, Dermatol Clin 2024), and named patterns including “pillow face”, “chipmunk cheeks” and “sunset eyes” (Lim, Wanitphakdeedecha and Yi, J Cosmet Dermatol 2024).

When the filler is hyaluronic acid, it can usually be reduced with selective hyaluronidase after ultrasound mapping. Collagen stimulators and fat are managed differently.
