A fat transfer that looked right once it settled can look heavier a year or two later. Often nothing has gone wrong with the graft itself. The fat that survived is living fat, and like fat elsewhere in the body, it grows when weight goes up.

Short answerGrafted fat that has taken behaves like the fat it came from, so it can enlarge with weight gain. This is called fat graft hypertrophy and makes up roughly 7% of adverse events reported after facial fat grafting. It cannot be dissolved. Stable weight is the first step; an excess that remains can be reduced from the surface with Time Freeze Laser LCLR® or removed surgically when it is large.

Why a graft can grow

Fat grafting moves fat from the abdomen or thighs into the face. Part of it does not survive and is absorbed in the first months. What survives establishes a blood supply and becomes ordinary living fat in its new place.

Living fat stores energy. When weight goes up, fat cells enlarge, including the ones that were grafted. A face that was given extra volume in the cheeks or temples therefore gains in those areas on top of the usual gain. A systematic review of facial fat grafting found hypertrophy in about 7% of reported adverse events.

Not every change in weight shows. A small case report followed a woman through pregnancy after a 7 ml facial graft and found the graft kept pace with the rest of her face rather than enlarging on its own. Larger grafts and larger weight swings are where the change becomes visible.

Signs that a graft has enlarged

  1. The face looks fuller than it did a few months after the graft settled, in the same areas that were grafted
  2. The change followed a period of weight gain
  3. The grafted areas feel soft and natural rather than firm or lumpy
  4. Fullness is uneven, for example one cheek heavier than the other
  5. The face is heavier when smiling, with fullness bunching in the mid-face

Illustrative image, not a patient. Grafted fat behaves like the rest of your fat, so stable weight protects the result.

Is it the graft, filler or ageing?

Many faces that have had fat grafting have also had filler. A heavy cheek can be graft, filler, a descending fat pad, or a combination. High-frequency ultrasound helps tell filler from fat and shows how deep the excess sits. The comparison is set out on fat graft or filler: what is overfilling the face.

What can be done

Stable weight first. If weight is still changing, any treatment aimed at the graft is aiming at a moving target. A period of stable weight shows how much excess remains.

Reduction from the surface. Time Freeze Laser LCLR® is built to target superficial excess fat and take it away gradually, while tightening the skin in the same session. The VF Lift – Vertical Facelift is added for patients who want more lift.

Surgical reduction. A large, deep and stable excess can be removed with microliposuction, which in Singapore is regulated as liposuction.

The full approach, including how areas that are genuinely hollow are supported, is on the fat grafting correction page.

“A graft that has taken is your own fat. It does not know it was moved, so when weight goes up, it goes up with it.”

Frequently Asked Questions

It can. Surviving grafted fat enlarges with weight gain like fat elsewhere in the body, which is why a stable weight before and after grafting is advised.

Grafted fat can shrink with weight loss, along with the rest of the facial fat. Rapid weight loss can therefore change how a grafted face looks too.

No. Hyaluronidase only dissolves hyaluronic acid filler. An enlarged graft is reduced with energy-based treatment from the surface or removed surgically.

The portion that will survive is usually settled within several months. After that, changes in the grafted area mostly follow changes in body weight.

It is one of the recognised complications. A 2022 systematic review of facial fat grafting found it in about 7% of reported adverse events.
