At the SAM Conference 2026, held on 25 and 26 July at PARKROYAL on Beach Road, Dr Sin Yong presented Second Gen Microwave Technology for Body Contouring, on the use of ultra-high frequency — microwave-band — energy for localised fat, cellulite and skin laxity, working with the HIRONIC MIGLOU™ platform. He shared the session with Dr Lim Tingsong and Dr Elias Tam, who presented on other modalities for body contouring.

The Session — Energy That Reaches Fat

The SAM Conference is the annual meeting of the Society of Aesthetic Medicine (Singapore), and the 2026 edition took as its theme Regenerative, Longevity & Holistic Medicine. Dr Sin Yong's contribution sat on the device side of that programme: what happens when energy in the microwave band is delivered into subcutaneous tissue, and what it can and cannot be expected to address.

The talk was built around a distinction that is often lost in device marketing. Body contouring platforms are frequently grouped together as though they were interchangeable, when in fact radiofrequency, focused ultrasound, cryolipolysis and microwave energy each interact with tissue by a different physical route. They are not competing versions of the same thing. They deposit energy in different places, and the clinical question is which of them suits the presentation in front of you.

Why Frequency Decides Depth

The physics is the part worth understanding, because everything clinical follows from it.

Tissue is not uniformly receptive to electromagnetic energy. How readily a given tissue absorbs that energy depends on its water and fat content and on the frequency being applied. Radiofrequency, operating lower down the spectrum, deposits a large share of its energy in the dermis and epidermis — which is precisely what you want when the target is dermal collagen and skin tightening is the objective.

Move up into the microwave band, around 2.45 GHz, and the absorption profile changes. Energy at that frequency is taken up preferentially by adipose tissue, so proportionally more of it arrives at the subcutaneous fat layer rather than being absorbed on the way through. Add contact cooling at the surface and the skin can be held down while the energy works below it.

That is the whole basis for using microwave energy on localised fat. It is also why it acts on the connective tissue between the adipose lobules — the septa whose tethering produces the dimpled surface associated with cellulite — and why a dermal collagen response follows as a secondary effect rather than being the primary target.

The HIRONIC Miglou Platform

The platform Dr Sin Yong presented on is HIRONIC's Miglou, which the manufacturer describes as an ultra-high frequency system delivering energy to selected tissue depths across both face and body. In Singapore it is distributed by IDSmed.

Dr Sin Yong is an International Key Opinion Leader for HIRONIC, alongside appointments with CLASSYS, DEKA, ALMA, FOTONA and LUTRONIC. HIRONIC's platforms also include the New Doublo 2.0, on which he has previously conducted physician training in Singapore.

A Multi-Modality Session

The session was not built around a single device. It surveyed body contouring across several modalities, with each speaker taking one. Dr Sin Yong presented the microwave case; Dr Lim Tingsong and Dr Elias Tam, both practising aesthetic physicians in the region, presented on other modalities.

That structure is the useful one. A single-device talk tells an audience what a machine does. A session that puts several modalities side by side forces the harder question — which one suits which presentation, and on what basis you choose between them. Sessions of this kind are peer education rather than promotion: the audience is other doctors, the questions are about parameters and patient selection, and the useful discussion tends to happen in the disagreements rather than the agreements.

One Modality, Two Platforms

Two days before this session, a DEKA delegation from Italy visited Dr Sin Yong in Singapore, and his practice was certified a DEKA Centre of Excellence. Among the DEKA platforms discussed during that visit was Onda Pro, which also works in the microwave band, using DEKA's Coolwaves® technology.

Working with microwave platforms from two different manufacturers is a useful position from which to teach the modality, because it separates what belongs to the physics from what belongs to a particular machine. Handpiece geometry, cooling design, depth selection and the shape of the treatment protocol differ between systems. The underlying interaction between energy at that frequency and adipose tissue does not.

That distinction — modality against implementation — was the thread running through the session.

What This Means for Patients

Very little of a conference session translates directly into a treatment recommendation, and it would be misleading to suggest otherwise. What it does affect is the quality of the reasoning behind a recommendation.

Body contouring is an area where patients are frequently offered whichever device a clinic happens to own. Understanding how the modalities differ — where each one places its energy, which presentations each suits, and which presentations are not suitable for energy-based treatment at all — is what allows that conversation to start with the anatomy rather than the equipment.

Suitability for any body contouring protocol is assessed in person. Response varies with tissue type, distribution, age and medical history, and no device is appropriate for every patient.
