Ultrasound-guided hyaluronidase means scanning the area with a high-frequency ultrasound probe to see where hyaluronic acid filler sits, then injecting hyaluronidase into that deposit under image guidance. The aim is to dissolve only the unwanted filler, at the right depth, rather than flooding a whole region. It is used for lumps, migration, the Tyndall effect and overfilled faces.

Key facts
What the scan shows
Filler deposits, their depth and their relation to vessels
Why it helps
Treatment can be targeted at the deposit rather than the whole area
Used for
Lumps, migrated filler, Tyndall effect, pillow face, and in complication management
Limits
Hyaluronidase dissolves hyaluronic acid only, not other fillers

Why image guidance

Filler does not always sit where it was intended, and it can persist for years: a review of 33 MRI studies found hyaluronic acid filler in the mid-face long after it was expected to have gone (Master, PRS Global Open 2024). High-frequency ultrasound shows the deposits directly, which was described for targeted hyaluronidase in cosmetic complications as early as 2016 (Quezada-Gaón and Wortsman, JEADV 2016). Dosing and safety follow published guidance (King 2018; Murray 2021).

Where it is used on this site: pillow face correction, under-eye filler lumps and nodules and filler correction. For the enzyme itself, see hyaluronidase.

Frequently Asked Questions

To see where the filler actually is, so hyaluronidase can be placed into the deposit and the rest of the area is left alone.

It is aimed at the deposits that are causing the problem. Whether to dissolve everything or only part is decided at the consultation.
